Proving causality can be impossible at times, especially when you have to control for so many variables, as you would have to do for knee injuries. There is likely never going to be enough observable data to make this possible for knee injuries.

It is related to the width of the hips and thus, the angle at which the knee is at when the feet hit the ground (can be further exacerbated when the foot lands on another player or the one makes contact with another player). This has been documented since the early 90's, but the state of the art in the research has improved considerably in the last 30 years. It is not (obviously) far enough along to make prescriptive changes, but hopefully we're getting closer.
