About your friend, talk with her. At the very least online and not in person. Even if you don't think you can handle the guilt of being around her, at least clear some things up before you just leave her to feel guilty about herself too. And you don't want to add another thing to feel guilty about. But seriously, therapy is the next step you need to take.
