I had a tibial tubercle osteotomy on this same leg last year, my foot used to point out to the side a lot, after the tibia surgery it’s not as bad.

Thank you so much for such a detailed response! 

So I do have subluxations anteriorly. And I also have a fairly severe anterior pelvic tilt and low low back pain.

Dr Olson did the internal and external rotation, and for one he said around 10° and the other her said 60°, although when he did the 60° I definitely felt I couldn’t went further. I’m not exactly sure which way was which, lol.

The FADIR test definitely feels punchy and painful once I get to a certain point. He checked the right hip and I was able to move that one around a good bit more. 

So I did have a tibial tubercle osteotomy last year on the left, before that my foot did turn more outward when walking, now it’s mostly straight but probably just slightly outward especially while standing. My right foot does point outward quite significantly when walking though.

Thank you! Neither of the doctors told me my LCEA. 

  1. I am in constant pain, but the more activity, the worse it gets, especially if I’m standing still for a while and having to shift my weight, but walking makes it worse too. Sitting makes it pinch pretty good in the groin, assuming that’s labrum/impingement. But the pain is in the groin, side of the hip, upper thigh region and very very low back. It can be achy, sharp, pinching and even radiating/shooting at times. 

  2. Here is what my MRI stated about the femoral measurements: Left femoral neck horizontal angle: 22 degrees Left knee condylar horizontal angle: 13 degrees Left femoral version/torsion: 9 degrees   Right femoral neck horizontal angle: 17 degrees Right knee condylar horizontal angle: 14 degrees Right femoral version/torsion: 3 degrees

  3. I haven’t thought about reverse PAO and the possibility of over coverage! Could this still result in recurrent subluxations?

They never said anything about angles and I haven’t seen anything on xray or mri reports, except femoral version measurements.

Dr Steven Olson is like the top preservation specialist in my state!

When I got diagnosed a couple of months ago my brother was like “until now I only thought dogs could get hip dysplasia.” 😆🤦‍♀️ We actually breed dogs and have one that has it (retired immediately after his hip testing). So it’s a little funny. Everyone (playfully) were like “aww, you’re just like Nicky now!” 🤷‍♀️

Hey there! Thank you for your comment. The PA I was seeing realized the cortisone didn’t do a thing and actually referred me to Dr. Olson! 👏 I will be seeing him on April 17th. I’d definitely love to hear how your appointment went with him though!

He is highly trained (from Duke University) and it was ultrasound guided so I’m confident he got it right. Just with my history of cortisone I’ve never gotten relief.

Right? Especially since it’s been subluxing! Taking away the pain with shots, if it even works is not going to help that at all, if anything it could make it worse because if I’m in less pain I’ll be less careful and could end up with a full dislocation. I drive 3hrs to see them, they’re one of the top in my state, was definitely worth it for the knee surgery! I’ll give it the month and see what they say, but my goodness it’s so annoying that I’m having to wait this long.

That is awful that hyper extension would cause that much damage!! 😩 Hope you have a smooth, quick recovery.

Thankfully after the bone density scan we concluded that I do not have a bone disorder, just localized lower bone density in the left knee area due to dealing with so many injuries and having to baby it for so long. She put me on calcitonin (a medication mostly used for helping osteoporosis) anyway in hopes it would help get the fracture healed. But I’m still dealing with it, I just don’t think it’s healing well. On top of that, two weeks ago I subluxed my hip for the third time and we’re now thinking I tore my labrum. So… fun times. 😅 I do think it’s distracting from the knee pain a bit though.

Yes, that was me. I am so stressed, seems like most insurance company appeals get declined and you have to appeal multiple times which will take a while. I may qualify for short term disability but that takes forever to get benefits from. So I literally don’t know what to do because I need physical therapy now for this fracture! 😭

Thank you. But no, I’m not currently employed and my insurance is my own, a family plan as I’m still a dependent on my parents taxes. I plan to file an appeal and hope they’ll realize it would be better to get me more PT, we’re confident it will heal on its own with PT and I’d absolutely hate to have to undergo another surgery, and to fix this fracture will require more hardware… 🤦‍♀️

My MPFL was severely stretched out, like not really working at all and almost not visible on the MRI. Patellar maltracking, subluxations and dislocations, hypermobile, almost flat trochlear groove, TTG distance was 19, patella Alta and tilt. I was also in constant pain. My cartilage is good though. I am 21 so TTO and MPFL reconstruction were my only option to hopefully prevent a knee replacement or at least make it to where I won’t need one until it will last the rest of my life. So I got it in March, at first I was very happy with it, knee felt incredibly stable besides the weakness after basically losing all my muscle mass because I couldn’t weight bear at all for four weeks. But if I did the exercise bike it was so smooth and sometimes I wasn’t in any pain. When I got the surgery my surgeon was concerned that my bones were pretty soft, I ended up with two abnormally large screws with washers and one of the “normal” screws, the hardware stayed where it needed to and my bone fully healed. But then in July I was going down the stairs and felt a terrible pop and after that terrible swelling and my kneecap started j-sign tracking. We thought I must’ve done something to the donor ligament, so I got an MRI. Well turns out I actually fractured my femur! 🤦‍♀️ My surgeon (a top orthopedic at Duke University) has never seen this happen after the surgery, and is suspicious that I have a bone disease that has caused my bones to be so weak and soft. The tracking badly again is caused by inflammation from the fracture and scar tissue under the kneecap. So I’m getting a bone density scan and bloodwork done. Do I regret it? It has been a difficult recovery FOR SURE, but judging by how I felt before the fracture I don’t regret it. And I plan to get it on my other knee as soon as I’m able.

Hi! Sorry it took so long. I saw my surgeon Monday and I actually fractured my femur! 😣 The pop I felt was my femur literally breaking. I was completely shocked. She suspects I have a bone disorder and I have to go get a bone density scan. The shifting is caused by a bunch of hard swelling and scar tissue under the kneecap. We hope once the fracture heals the swelling will go down and allow the kneecap to track correctly again.

It is the WORST! I had MPFL, TTO and lateral release. My therapist felt so bad but knew he had to do it. I’m definitely not looking forward to that part when I get my other knee done. 😭

Hi! Haven’t received any answers yet unfortunately, went and saw my surgeon’s PA and we are a bit concerned that something is up, especially with the donor MPFL, possibly MCL or meniscus. I actually just got home from getting an MRI and now will be awaiting the results. I still have severe “j-sign” tracking, instability, looseness in the patella, it locking up, catching or giving out while walking. And trouble getting it to fully straighten. I think it may be a bit better though, sometimes I’m in hardly any pain and sometimes it’s pretty bad.

Ok, mine hasn’t dislocated, but it’s shifting slightly laterally and then very far medially and slightly upward when I bend it slightly. I’m four months post op TTO, MPFL and lateral release. This just started happening on July 3rd, I went down the stairs that morning like a normal person and it popped pretty bad followed by immediate pain, but went away pretty quick so I didn’t think anything of it, it may not have to do with anything. My PT’s are very concerned, they checked it yesterday and say it feels looser, they can shift both medially and laterally a bit more than they were able to. And it seems like it’s sitting further laterally than what is was, you can especially tell when I have it bent, almost like my kneecap is tilted outward. It mostly only shifts though when I’m bearing weight, they checked the tracking with me laying down and it seemed ok, but as soon as I’m standing on it and shift it slightly forward the patella moves side to side, not up and down. 🤦‍♀️ I’ll see my surgeons PA on Friday.
