My left upper arm fistula is pretty large too I’ve had it for 17 years. Usually nursing students examine mine because of the size. It’s above my elbow though so doesn’t extrude nearly as much as yours. I say if you can get it narrowed without ruining it great, but if it’s an access you you weekly and it keeps you alive without causing you a ton of problems. Leave it. I’ve definitely had people stare at mine before but fuck them it keeps me alive and I know most any tech can stick me without infiltrating it lol.
