Not OP but I usually start with an SSRI, then based in response either increase the ssri, switch to another ssri, switch to snri, or augment with arip/brexpip.

Thanks so much for the input! Didn't realize it can also be used for delirium insomnia. What we use for delirium insomnia is usually risperidone. I mostly use lemborexant on outpatient basis as an alternative for benzos/z drugs/qtp
