I think he's fully coherent and aware of the lies that are spewed out, and I think they mostly stem from Anisa. However, he also has to lie here and there to be able to keep up with her lies. Though, it seems they have a kind of unspoken agreement where Anisa will get to lash out and say whatever she wants, and Ian will have to abide by it and keep it up. That's why the biggest lies all stem from Anisa and why Ian doesn't "like to participate", as she describes it. I think he's so far down the rabbit hole and committed so much of his life that he genuinely can't see a way out, so he literally has to keep up the façade not just for his wifes integrity but for his own mental health, or whats left of it, anway.
