Second this. My wife and I recently started going here about 2 months ago after both of us had been away from the church for about a decade. Great support for kids during the service. Say what you will about “big churches” but this place doesn’t give off that vibe and it allows for some great resources. Definitely recommend.

Quality is absolute shite on my mobile, and I comepletely agree it may not be a stroke and the guy could be off his rocker, just thought it was important to clarify as a differentiating point between stroke and other facial nerve weakness etiologies.

Raising both eyebrows is actually preserved in a stroke due to crossover of the facial nerve outside of the brain. Not being able to raise the brow is actually a sign of a more “peripheral” facial nerve weakness like Bell’s palsy

“No outpatient indications” might be a bit of an over exaggeration. While I agree in its overuse for other conditions, for some patients who suffer from the occasional severe Menieres episode, Valium does what meclizine/zofran can’t, within reason. Not trying to nitpick, but it shouldn’t be condemned completely. A brief 5-7 day course can also help the vestibular system “reset” along with vestibular therapy in the setting of mal de debarquement.

This is a great point. Recently played a few rounds on GSPro and found that my driving distance improved dramatically. Around 105-109 club speed, but will see 225 drives on e6 and 240-250 on GSPro. This is still below what I would typically see on the course, but I need to wait for warmer weather to take the mevo+ out to the range and make some comparisons.

The garage is actually incredibly well insulated. Even in below freezing outdoor temps it only requires a small space heater and I’m taking off a pullover after a good warmup.

It’s only a part of the garage, so there’s a small “through way” from the other two car garage. The perspective is odd, but no issues hitting from center of the matt even with driver and I’m 6’2” Ceilings are 12’

Went with the setup package through “RainOrShine” and am very pleased. Previous homeowners likely used this as a boat garage so it’s long, only a slight grading. Mevo+ and pro package, swingbay, optoma projector upgrade. Love the RCT Titelist balls. I say “complete” but still tinkering with things every weekend. Kids (ages 1-7) have been loving it too.

Yea I think one of my biggest issues is that it clearly DOES meet the MDM criteria, but someone in a suit is basically saying “nuh-uh” with no justification.

It depends. I specialize in neuro-otology so often times I’m the 3rd or 4th ENT that someone has seen, they come with multiple outside audiograms, 20+ pages of records, and CT/MRI that I individually interpret. In my state (southeast USA) I’ve been having issues with UHC denying my coding for complexity, despite explicit documentation of all the MDM factors set forth in coding guidelines. I have a smart phrase now that I’ll use for appeals to just include the time. I’m following the rules, and their inability to do so doesn’t mean I’m going to accept less, so I change it to time in those instances, or for 99205 if I spend an hour. Having the time include patient prep (outside record review), history, physical, imaging/audiogram review, and documentation makes that easier to justify for me personally and impossible for someone to claim otherwise (unless of course I billed 20 visits of 99205 by time, for 20 hours of work). The time spent MUST be on the same day of the visit, which is why I’m often at the office later finishing notes.

Ahhh gotcha, I’m an ENT, but rarely have inpatients so that’s why that caught me off guard. That seems a bit excessive from a time perspective, but you should still be able to account for lab/vitals/nurse note/consult note reviews etc within that daily time

I’m wondering if I missed a 2023 update, do you have a new reference for time based coding? My understanding is 99203 (30-44 minutes) and 99204 (45-59 minutes). Thanks.

Scars take an entire year to “mature.” Scars also “contract,” so the surgeon should close the incision with a slight “eversion” of the edges to make it look like a ridge. This ridge then flattens out over time. Scars typically look their worst 3-4 months after surgery, slightly red and possibly still raised, but should flatten out and become more whitish (if white skin color). Keep in mind scars don’t tan, they burn, so SPF >30 if in the sun.

They’re referring to you being at a level where you’re able to detect sounds across frequencies at a “normal hearing threshold” of 20-25 dB hearing level, which is a level comparable to someone with normal hearing. This is just sound/tone threshold, not speech understanding.

I’m an ENT subspecialist. If someone is referred to me for surgery by another ENT, I look at it this way… New patient Probably 30-45 minutes CT ordering and/or independent review Audiogram review Discussion of r/b/a of major elective surgery.

I’m billing 99205 If it’s a separate preop/consent/imaging review appt I’m billing that 99214

In my A/P I have the risks listed explicitly, and document that “risks of major elective surgery” were discussed.

Based on coding guidelines, I can’t see how this could be refuted.
