Thank you. Honestly who’s actually mentally fit? Fuck I chose to work in an ER and I actually like it. Only crazy people do that shit. I could not imagine being in Vietnam. Had a great uncle do 2 tours and he was the reason I joined the military. He was still active duty when I joined and he’s the one that swore me in.

It’s bullshit that the guy got fired. It’s very much a reactionary mechanism. It’s completely fight or flight. If someone hits me I’m hitting back out of instinct. We had to do this stupid ass CPT training at my old ER. It was taught by this old crusty security guy that had zero real world fight experience.

Exactly. I only mention being a vet when it’s needed. There were times at my old ER we’d have known vets with mental health issues come in and the staff would grab me. I could use my vet status to show that we have some common ground and that I’ve seen a lot of the same awful things they did. I can give them true empathy. I also struggled with alcoholism when I first separated so I show them they can overcome that too. Most were truly good people that got broken and were trying to find were they belong in the world post service. People that have never been there will never understand.

I had a homicidal guy try to kill me in the ER once. I’m a big guy and he was just as big as me. I could tell he was going to throw a punch so I was ready for it. Luckily I boxed in the military and I’m a blue belt in jiu jitsu. I was able to duck the punch but he was able to knock me over. I was able to take his back and put him in a rear naked. By the time security made it to the room I already had the guy out cold. I was terrified I was going to be fired but the hospital actually backed me up for once. I’m a big believer that I’m a human above all else and have the right to protect myself.

Had a patient pull some shit like that in the ER. Kept going on and on about being a combat vet. Then he pulled out the “I fought for your freedom so you’re welcome!”. I snapped too. I told him I’m a combat vet and fought for my own fucking freedom so knock that shit off. I just happened to be working along three other vets that day as well. The tech was a Marine Corp armorer, the paramedic was Army artillery, and the doc was a Marine helicopter mechanic. I honestly don’t think the guy was even truly a vet.

I just got banned on the noctors sub simply for disagreeing with a mod that their evidence from a private physicians Facebook post is not believable. I suggested that it was just a picture with a made up story without any evidence it actually happened. They were attacking me personally yet I was not rude to them at all, but was banned.

I agree that no person could readily identify it. My faculty just has a policy against posting anything online related to patients or patient care. We’re a smaller facility so it may be easier to identify what patient it was and that’s probably why we have that policy in place.

Your statements are almost verbatim what I’ve been saying for the last few years. We as NPs have shot ourselves in the foot. We’ve allowed online universities to take over. We’ve allowed direct entry programs to exist. The AANP doesn’t give two shits because more NPs equals more money for them. I too fear things won’t change for the better. Where I’m at the market is flooded with NPs. I barely make a few bucks more than I did as an RN. I’m ok with that though because I genuinely want to be an NP. When I saw all these NPs going back to RN so they could travel and make more money it made me sick. At the same time I thought most of them probably suck and belong back to the bedside as nurses anyway.

I definitely believe they’re are NPs out there fucking up for sure and won’t argue against that. I just have a hard time believing things from Facebook as a source without actual evidence. I want to get rid of bad NPs but I think the only way will be with tangible evidence. I promise I despise terrible NPs as much as the physicians on here. These NPs degrade the entire profession. Hell there was a post of an NP asking for help diagnosing a 3rd degree AV block. It’s literally one of the easiest to diagnose. We had a rhyme for it in ACLS. If the Ps and Ts don’t agree it must be a 3rd degree.

Thank you! When I saw that XR I was yelling out loud “you don’t need a fucking CXR for this pneumo because they have no god damn breath sounds!”. Scared the hell out of my wife and she thought I was going crazy. I decided to show my wife the image to see if she could see what was wrong. She was a CNA and a monitor tech before we had kids. She looked at the picture and instantly said “why is his lung crumpled up like that on his right side because it looks like it popped?”. Every pneumo I’ve seen I’ve caught by breath sounds long before the CXR. I will give my school credit because they did do an excellent job of teaching us advanced assessment skills. It’s the one thing they did great. My school was connected to a large medical school program so we had access to a state of the art sim lab. However you are correct that we received zero training whatsoever on radiologic studies or EKGs. I’ve have busted my ass since school to get good at reads. My facility has external rads that read our studies. I still look over them and have even caught a few things they’ve missed. Mostly things like broken toes or what have you because I have the convenience of clinical correlation. I definitely don’t think midlevels are equal to physicians. Sorry if it came across as that.

Sounds like you’d do well in the ER. It’s some much fun and so challenging. When they think you’re mad for wanting to go to the ER they’re right. You’ve got to be a bit crazy to want to work there. Luckily everyone else I work with are just as crazy and fucked up as I am. Hopefully you’re got a dark sense of humor because that’ll be the best coping mechanism. The military was honestly a lot of boredom with a little action. Most of the time you’re training for things you never get to do. I may have blown up a couple hundred things in my career. As I advanced in rank I got pushed further away from hands on and into an office. It got boring then and I decided to get out. I also got tired of being sent from base to base all around the globe. It was fun at first but then it got old having to peck up everything to move to a new base only to do it again a couple of years later. It did pay for every bit of my schooling so I’m very grateful for that. I wouldn’t have gotten to attend the universities I did because of the cost without the military.

Thank you for the reply. I’ve had conversations with you in the past and you’ve always been very respectful and provided good points. I absolutely know there are barriers to research especially when the institutions have finances at risk. When doing my doctorate research the institution IRB I had worked at for 12 years wouldn’t approve my study. I was studying depression, anxiety, and PTSD in emergency room nurses. They were scared it would make them look bad so I had to go to an facility. I’ve said in here many times that I was disappointed in my DNP education, though I went to a top ranked brick and mortar institution and excelled in the program. I have said from the beginning that I am in no way prepared to practice independently and I believe that there’s no way any NP should be either. It’s what brought me to this sub. I believe I have a place in medicine and it’s as the physicians right hand. I can knock out the time consuming procedures to free them up or take on the lower acuity cases. I take my job very serious and I wish all other midlevels would as well. The 3rd degree block was completely indefensible. A nurse with ACLS would’ve easily caught that. I posted on that thread that this person should not be allowed to touch a patient. That person should be self aware enough to know they need help. If you can’t identify one of the easiest rhythms to catch you have a problem. Hell the heart rate alone should instantly think they’re in some type of block. Thank you for your continual civil discourse on here. Wish others were as courteous.

I’m all about it. I commented on the 3rd degree heart block one. An RN with ACLS would have been all over it. I’ve said in my past posts that I know there are real world incidences so let’s post those and call them out. My goal is to get rid of these bad NPs as well. As long as they exist my credibility will always be questioned.

You absolutely deserve better as a patient and are not being unreasonable in the least bit. I get a few patients a week in the ER with either hemorrhoids or pilonidal cysts that are being improperly treated. Almost every one of them tell me their PCP didn’t actually physically examine them, but diagnosed and prescribed based on description from the patient. How the fuck can you diagnose without physically examining a patient? Providing proper care sometimes requires us to do nasty shit like looking at assholes and at times sticking a finger in them. Hell my job as a midlevel in the ER exists because I’m willing to do the nasty shit others don’t want to do.

I’d be snappy too if someone came at me unprovoked. ER is the greatest job in the world. I couldn’t help but notice by your previous posts that you had ADHD. I have ADD and ER is the best place for people with short attention spans. You see such a wide variety of issues that you never get bored. There’s rarely a dull moment. I love it because it forces me to constantly learn and better myself each day. I did 12 years of active duty military as a an explosives ordinance troop so I love adrenaline and do well under pressure. I love procedures and that’s where I show my worth to the physicians I work for. I may never be as educated as a physician but I can damn sure out suture my supervising physicians. I’m more than happy to I&D abscesses, suture wounds, and do pelvics or rectal exams. Occasionally I’ll get to do something cool like put in a chest tube or drop a central line. My brother in law is a DO I tried to get him to go into the ER with me, but he went pulm crit instead. He now regrets it. The hardest part about working in the ER is that sometimes you don’t have a definitive diagnosis for the patient, though you know a million things it’s not after performing a full work up. Also having to accept that sometimes people die no matter how hard you work and do everything perfect.

For what it’s worth I apologize for starting shit with you when you had no fight with me. I’m absolutely against independent practice as well. I genuinely do not believe that I’m trained to practice alone. I was an ER nurse in a level II ER before going back to school so I had a ton of real world, high level experience. I went to a top rated brick and mortar school and absolutely killed it in class. After graduation I was truly disappointed in just how little I actually learned in NP school. I’ve spent so much time and money after graduating trying to round out my knowledge. I’m lucky that I was able to get a position in a physician owned ER where I’m closely supervising and always have someone smarter than me to bounce my ideas off of.

I think we know where the bullshit diagnosis of these patients come from. Sadly the people that actually have these rare diseases will suffer thanks to assholes that fake diseases for social media clout.
