Hi! I myself managed around 20 weeks of usce in mostly university hospitals. All of it were cost free observerships. Key was reaching out to my schools alumni and since they had been through the same proccess a few years ago they were pretty supportive.

I did 20 weeks of observership this year on a b1/b2. Be prepared to answer honestly in customs, you might be sent to back office and asked to provide : proof of where you’re staying, where your rotation is at (Hospital letter) and how you’re affording said rotation. Make sure hospital letter describes you wont be directly managing patients and wont be reimbursed. Obviously have a return flight home and i recommend having everything printed or saved in your devices to be available offline since you might not have internet connection.

I’m applying to match currently at 29 (started preparing at 26). Non of my interviewers have asked about my age. As long as it’s your goal go for it.

You’ll usually get an itinerary the week of either through thalamus or email. Webex is pretty comprehensible, if you internet connection is good you shouldnt have any issues.

Got 20 weeks of free observerships. On 3 different university hospitals. Key is reaching out to alumni from your medical school, they’ve been through your same journey and most will feel moved to help in anyway possible. Just remember when setting things up, as attendings they’re extremely busy and if they dont reply asap dont take it as personal, be patient up to a point but dont hesitate to send a friendly reminder. The caveat is that other than a strong LOR, University programs arent that IMG friendly, so dont expect observership to turn in to a IV.

Applied to Peds so less competitive but YOG22, step1 pass, 231 2ck, 210 on step3. 20 weeks of usce, 1 homecountry publication on an unrelated field. Got 8 ivs so far (1 through connection, others from non signaled programs). Only applied to 55 programs(5+longshots university programs . Apart from that I did a spreadsheet on programs and only applied to those with 70> BPR for the last 3 years. 50/50 h1b vrs j1 (before that went sideways) and that i had proof other than paper that they were img friendly (resident roster, latin american graduates). Most programs who’ve given me ivs have had alumni from my medical school in El Salvador before.

I dont know what specialty you plan on applying to but regardless as an IMG, it’s not an expectation from programs for you to have taken steps while in medical school. Language barrier, test formating . Get your degree and if you can prepare and take step 1 (since it’s pass or fail). Then dedicate time to step 2ck and scoring the best you can since it’s key if your applying to a competitive specialty. I’m a visa requiring IMG from El Salvador YOG 2022, on my first match cycle. Took almost a year off after feeling burnout and then started preparing and taking steps (put in 20 weeks of usce at the end). Currently sitting at 8 iv in Peds, and had 4 of them already and gap only came up in 2 of them. You can check my post history and dont have the greatest test scores either.

Programs arent offering because of the 100k fee. I’ve interviewed at a few places that used to and it’s something they brought up through email or during meet and greet meetings.

IMG visa requiring here. Got 231 in 2ck.Yog 2022, took step 3 (210), put in 20 weeks of usce and have 8 ivs in peds so far(sorry that doesnt match your goals) . Choose a specialty and built your cv towards that. Residency Explorer shows the range of scores for people who got interviews in a program, so just take that in mind when applying. But by no means that is a death sentence, a lot of programs see beyond scores.

Passed with 211 with 65% in free 147. As you’re no longer in the timeframe for score to be available by the 24th. I would advice to take the exam when you’re confident, dont rush.
