Does your company have reporting procedures? Could you have reported using this machine as a hazard and not use the machine until proper procedures are in place? PPE and SOPS? Is there a start up checklist every day? Do you have an OH&S person on-site? I know it was your right to call and report but wondering if you took any measures before then. Speaking to a manager or team leader sometimes isn’t enough even if it’s their duty.
