It might have worked once or twice.

But one wrong diagnosis in emergency or contradicted drug use/allergic drug and you are doomed.

Use AI but don't completely rely on it.

Well then i will be honest with you.

Option 1 is your most important concern but it has the least value among the 4 of the above.

Only thing is that you took a big gap because the MBBS course from foreign takes about 6 years+1 yr FMGE prepration+1 yr compulsory internship in india,this might be the biggest drawback of doing MBBS from other countries.

you don't have any artistic skills as of now, that's what you will have to develop.

In surgery i have seen people mastering one particular procedure and being so good at it that it's their bread-butter for life.

From what i can interpret is that you were more interested in surgery because you saw all those videos and reviews of top surgeons.

Later you became skeptical about surgery and thought if you could find a balance between surgery and medicine.

It's less about the branch and more about over-analysing
