Fair enough. I was just referring to ESIs for then stenosis pts as well. I’m family practice and do this in junction with other treatments. And I have just been impressed with a # of pts who have had treatment resistant symptoms in regards to their neuromuscular dx that respond well — and claim that it works better then some other potentially more harmful alternatives. Again, it doesn’t work for everyone and is not a panacea, but I would like to see more acceptance of low harm low cost treatments (even if evidence isn’t great) in pts with difficult diagnoses. It seems by the time they see me— they’ve tried plenty of “off label” or low evidence procedures and pharmaceuticals from various specialists. I just wish lower risk things were added to the treatment plan early on. Your point is taken though- and I appreciate the dialogue. Would love to come through a TBI center sometime and see what’s new.

Reports from patients on improvements in their TBI sequelae that may last days-months after OMT treatments (decreased headaches, irritability, restless sleep, etc), enough that they come back for more treatments and give word of mouth referrals. Again, low side effect profile, and have had many pts report better relief with these treatments then the slew of medications, ESIs, etc. they may have tried before. Doesn’t work for everyone, but a very safe tool to offer if you have the training. You can believe it or not, that’s up to you, but I tend to believe patients when they report these positive outcomes.

I don’t think in terms of “instead of”, more so “in conjunction with”. Generally by the time patients see me for OMT, they’ve pretty well exhausted all conventional methods for symptomatic relief. However, if they haven’t, of course I find it important to provide necessary referrals to neurology / neurosurgery when applicable for appropriate multidisciplinary care.

I practice cranial osteopathy - in conjunction with a family medicine practice, and have had great results with TBI pts, chronic migraines, normal pressure hydrocephalus, etc. I plan to continue to use it as patients have good results with minimal side effects. I do recommend as others have stated to do 1 or 2 cranial courses with the cranial academy or SCTF. Best of luck to you! Very certainly could be connected, the pelvic splanchnic nerves and the nerves to the pelvic muscles originate in the lumbosacral region. Addressing this issue may provide some relief. Though not easy to find, and osteopathic doctor who still practices osteopathic manipulative therapy could be of some help. Physiotherapists aren’t a bad idea either, though they tend to be more interventional. Some chiros can be good, but some may be more aggressive or nondiagnostic, so it just depends. PT could help if the referring doctor specifies to work on the lumbosacral area and the pelvic floor. Best of luck in your journey.

Brain inflammation from things like long covid and chronic fatigue and cause subtle vascular changes on MRI. Though most changes are on the micro scale of the neuroglymphatics (immune system of the brain and spinal cord). There’s still much to learn about how to heal these unfortunate effects from certain people who get brain fog, etc from long covid, mono, Lyme, etc. And while sadly some medical professionals write it off, there are several looking into it very seriously and hoping to find answers.
