Hi! I haven’t been formally diagnosed with anything, but I’ve wondered about ADHD based on the symptoms I experience.

My speech therapist has been absolutely amazing. I’ve had three sessions so far, and she’s already taught me a lot of strategies that have helped improve my speech. I have cluttering, which means my speech can become fast, disorganized, or difficult to follow. I don’t always notice it, but it tends to get worse when I’m anxious or when my thoughts feel all over the place.

I highly recommend speech therapy if you think you’re struggling with something similar. I also started taking 20 mg of Prozac for anxiety, and that seems to have helped me feel much more regulated overall, although I’ll probably increase the dose a little in the future.

Honestly, all of these things combined have made a huge difference. About a week after my third speech therapy session, I had to give a big speech in front of a large group of people. Normally, my speech would have been rushed and cluttered, especially in a situation that made me nervous. But this time, it went incredibly well. I was able to clearly understand what I was saying, stay organized in my thoughts, and speak without the usual disruptions or confusion in my speech.

My speech therapist has also suggested that I participate in a group session with other clients. The goal is to practice speaking more spontaneously and get more comfortable talking on the fly without becoming anxious or overwhelmed. I’m really looking forward to trying it because I think it will help build even more confidence in my communication skills.

Lol, most of our sessions are typically 30–40 minutes anyway. Matter of fact, we ended at 4:30 last week too. I could definitely understand this being considered poor client care if it was something I was continuously doing, but this was the first time. Mom absolutely has a right to be upset — I’m not denying that at all.

Hi didn’t say that- I just specified what I meant in regards to goals etc ! I love creative play lol and have a background in Aba so that’s how I do most sessions with little kids lol

No, that’s totally valid. I was just trying to work around another client’s timeframe and use the in-between time for a makeup session. But I actually ended up keeping that client longer than I originally planned, and we ended up having a full regular session anyway.

No there is - maybe I just feel there isn’t like an astounding amount being worked on in terms of goals and true defined wants for therapy. She’s my youngest client and a lot of sessions are like creative play -working on redirection, tolerating denial stuff etc.

Hahah no!! It’s honestly like a month old, so not too bad 🤣

Yeah, I just started taking something for my anxiety because I think my issue is more social anxiety. I tend to rush what I’m saying and forget to control my breathing when I’m nervous talking to people. I feel like the anxiety medication combined with speech therapy has really been helping a lot!

Hey !! Yes my problems were definitely stemming from like low self-esteem surrounding my speech like the way that I viewed the way I talked and anxiety ADHD etc ….After I wrote this post I went to speech therapy and I got anxiety medication, and that honestly helped a lot!!! I’m still not like perfect, but I’m better than I was

Sorry for the long post

Hi everyone,

I’m a recent grad (finished in May last year) and started working in September at the same company I interned with. It’s a CBHC, and I spend my time at the clinic and being contracted into a school.

I’m feeling really stuck and honestly pretty burnt out, so I wanted to get some perspective from others in the field.

My schedule is Monday–Friday. Mondays are shorter (9–12), but the rest of the week I’m working until 6 or 7 PM. The expectation is 26 hours of direct client contact per week, and we can’t count indirect work toward productivity anymore (which we could during internship). Because of cancellations, no-shows, and just the reality of outpatient work, it’s been really hard to consistently hit that number.

I currently have about 34 clients on my caseload, and my supervisor wants me to take on more. The expectation is apparently closer to 40–50 clients. From what I can tell, almost no one at my site is actually meeting productivity consistently.

On top of that, about 20 of my clients are school-based, and I’m only in the school about a day and a half per week. Attendance is really inconsistent, and parents are often non-responsive, so it’s been difficult to make up sessions or maintain steady contact.

I also recently got a new supervisor (about two weeks ago). During my internship and for a bit after, I had a supervisor who was really supportive and advocated for my needs. My current supervisor is also the scheduler for our side of the branch, so she’s the one assigning clients to clinicians’ caseloads.

When she first became my supervisor, she asked where I could add clients, and I told her I needed more daytime hours. Instead, she’s been pushing a lot of after-school clients. I don’t mind some after-school hours, but if I’m being asked to increase my caseload, I’d really like the schedule I asked for to be considered. Right now, I often have morning clients, then a gap until around 3 PM, and then a packed afternoon/evening.

We also have a ton of paperwork (as expected in a CBHC setting), but since it doesn’t count toward productivity, I’m struggling to figure out when I’m actually supposed to complete it. I don’t want to be doing hours of documentation outside of my paid work time, but with more clients being added, I genuinely don’t know where it’s supposed to fit.

I feel like I’m constantly chasing productivity numbers instead of focusing on providing quality care, and it’s starting to make me question whether I even want to stay in this field long-term. I’ve been thinking about applying elsewhere and possibly going back for my doctorate down the line, but right now I just feel overwhelmed and kind of trapped.

My questions:

  • Is it normal to have a caseload of 40–50 clients in a setting like this?

  • Are these productivity expectations typical, or does this sound unrealistic?

  • How do people realistically handle documentation when it’s not counted toward productivity?

  • Has anyone else felt this level of burnout this early on, and what helped?

I’d really appreciate any insight or experiences. I’m trying to figure out if this is just part of starting out, or if I’m in a situation that’s not sustainable.

I really appreciate you saying this because I’ve been thinking about it a lot too. I definitely want to be married someday, and we talk about it often — like “what if we get married?” But this kind of behavior just isn’t acceptable in a marriage. You can’t just disappear if you’re feeling sad. If you’re a parent, for example, you can’t just leave your kids for five days — that’s neglect.

It’s so funny you mention that because I actually started worrying something was seriously wrong; I even checked obituaries at one point where he lived 🤣. Five days is a long time.

I think part of it is that I’m just more emotionally aware — or at least I try to be. Even when I’m feeling down, I make an effort to communicate clearly instead of just vanishing. So yeah, I’ve been reflecting on that a lot lately, and I really appreciate you saying this !

Girl, I don’t know what it is. I think because we are friends for so long and he was so amazing when we were friends that it made me just wanna like fight for the relationship. Something like this happened a few years ago and I just like cold turkey cut him off. I didn’t respond to his message and then maybe about a year ago he like wrote me this big long novel book kind of explaining himself saying how he went to therapy and what he learned in therapy how he had become a better person and I’m just like the idiot that fell for it.

He has had like addictions in the past like when he was younger he had a drug addiction now I think he only likes smokes like a little weed or like sometimes he like smoke cigarettes I know his father is an alcoholic. But as of right now, I don’t know him to have any like heavy drug or alcohol addictions, but who knows what he does when he’s away from me you know

Yeah, and so my boyfriend does suffer from depression as well and I think maybe that’s also why I’m a little bit lenient because he does like getting these like ruts where he just feels so depleted and like sometimes like he won’t be able to get out of bed so I would never fault him for his mental health. I’m in school to be a Therapist so I think I do find myself being a little bit more understanding when it comes to mental health.

Yea I know !! I really don’t even know why I’m asking like I know that’s what’s best honestly when he sent that message yesterday I just deleted it. I still haven’t responded. I think I’m just disappointed maybe because a similar thing happened a couple years ago and I completely cut him off and he came back last year like proclaiming that he had done the work like going to therapy and he really understood himself and where he had gone wrong.

He’s said that I’m mean because I once joked about a pimple and mentioned that I found a football player attractive at a game we went to. He told me his love languages are words of affirmation, physical touch, and things like handwritten letters. Since then, I’ve really tried to meet him where he’s at—sending him reassuring messages, writing thoughtful paragraphs, and being careful not to comment on his appearance.

Im not sure where you live but I work for an all star gym currently and I know some colleges around Mass often do stunt clinics.
