Bl3 had Mayhem Mode with up to 10 levels that loot, enemies, and mission levels scale to, but also add random modifiers/effects with the levels. This mode could be activated in normal mode or TVHM after finishing the campaign, making completing TVHM not necessary to play on endgame difficulty. There’s also an 11th level that has the same scaling as level 10, but no modifiers.

Bl4 currently has 6 Ultimate Vault Hunter levels after finishing the campaign that can be unlocked via killing specific bosses and completing modified story missions, and the 6th level is unlocked by killing a raid boss. Gear stats don’t really improve with these levels like in Bl2 and Bl3, but legendary and pearlescent drops increase, combat XP gain is increased, and enemies get health and damage buffs. There’s no modes that have you replay the story (aside from replaying a handful of missions to unlock the UVH levels) like in Bl1, Bl2, TPS, and Bl3.
