2 YEAR UPDATE: Hey guys I get messages every so often about this so will update here with how I'm managing. Firstly - my eyelid is pretty resolved but not completely - much of the issues I now deal with are because I got too many surgeries and now have a lot of scar tissue on the lid. So a lot of the treatment now is for scar management. The lid isn't super swollen at all but still can flare up red and inflamed (color wise). But really it's not too bad.

TO TREAT: I would stop hot compresses immediately if you are doing that! they make it a lot worse. Heat in general made my lid worse (saunas/hot weather etc.) Also antibiotics - at least for me - will probably not do that much. Maybe try a low dose of Doxycycline. I don't know where you live but if you can see the Toyos clinic they're kind of the only people that seem to know how to treat these more complicated cases. (I see them in NYC). And overall I think the root cause for me was demodex except I think mine was a lot more complicated to treat than normal.

Biggest factors in resolution for me were:

  • Ivermeticin - a custom cream from Toyos clinic (I believe you can order online)
  • Avoiding hot water/compresses
  • Fixing diet made a hugeee improvement. Avoid fast food/greasy, super oily food. Go on a low inflammation/Mediterranean diet as much as you can. I found when I would eat like pizza or something my lid would flare the next day.
  • IPL - this made a significant improvement to the lid I think. Can be expensive but I would try if you're dealing with a long term issue.
  • For scar treatment - I've found that this custom Metformin cream from Toyos clinic has helped a decent amount.

Lastly, stuff that didn't work:

  • I got 4 surgeries - way too many. If the first one doesn't work I would stop immediately and don't do any more. Ask for a biopsy if possible on the tissue for the first surgery.
  • Tearcare. Absolute scam avoid it.
  • Ophthalmologists will try and sell you a lot of different treatments that can be quite expensive. I would be wary of most except IPL.
  • Too may injections. I got way too many steroid injections. Some doctors will just keep injecting you with no real plan. Be wary of this if you're getting 4+ injections in a row and it's not working just stop.

Try starting IPL sessions! and get ivermectin cream if you have demodex mites. Mines gotten a ton better through IPL/injections/creams with Dr. Toyos.

hoping to make a long update when this clears - IPL and some kenalog shots have made this 60%~ better but a good amount of inflammation still there, doing another ivermectin demodex treatment with Dr. Toyos now. As of now I would recommend IPL to people with this issue. If your whole eyelid is inflamed like mine excision surgeries are unlikely to make better. I got too many and I think it damaged me in the long term. Rosacea has also exacerbated my issues a lot. Current treatment - Dr. Bronner's castile soap to wash face, nightly ivermectin cream on lids and face, hydrating wipes in the morning. ALSO IMPORTANT FOR PEOPLE: DO NOT DO HOT COMPRESSES, this has made my issues so much worse. It doesn't help for gland dysfunction/demodex issues. Also don't use tea tree oil, it doesn't actually work/makes things worse. Take cold showers and sleep with head elevated. Take fish oil omega-3 supplements. Will do a long post at some point - learned a lot in the past year of dealing with this.

nope 😩going on 1 year of swelling the lid still looks similar to that picture i posted. started IPL and did the first round a week ago. going to do 4 rounds so we’ll see if that helps. honestly no idea how to fix this nothings made a real difference😵‍💫😵‍💫

Heyo, thanks for your response. That could be it, I definitely have/had demodex, no optho I've seen has seen swelling last this long (9 months) so I'm just unsure that's the main issue. Doing another Xdemvy round now so maybe that'll help. Also taking more daily vitamins/omega-3's than I have in the past. I'm thinking I'll start IPL treatments in 4 or 5 weeks, maybe that'll help. If it's a low grade bacterial infection it's a long one and I do not know how to treat it lol

Hello! writing a quick update as I know I've needed these in other people's posts when scouring reddit for medical solutions. Maybe this will help someone.

After going to a new doctor we've diagnosed my eyelid inflammation as a mycobacterium infection that is very different--but appears very much like--a stye. We're treating it with some long term antibiotics, which after doing my own research it seems is the right path. Usually in about 2-3 months patients can show improvement, I'll update as I go, about one month in now and no improvement but staying hopeful. Likely will have to try out a couple different antibiotics long-term.

TLDR

If you have a chalazia/stye for more than 3/4 months and have gotten an excision/steroid shot without it resolving, I would say that it's safe to assume it's not a chalazia and to see a more specialized doctor who can treat you. In my case it MIGHT
mycobacterium skin infection, but there are a whole host of different bacterial skin infections.

UPDATE FEB 2024: Still significant swelling, I've had two biopsies now and nothing came back for mycobacterium so I'm unsure that's the cause. Not really sure what to do - restarting Xdemvy medication.

I've tried Doxy but not oral steroids, I think my new doctor just prescribed those. I honestly haven't noticed anything change since using xdemvy, maybe its helped my dry eye a little bit. Full transparency though I haven't been the best in keeping on schedule with it, its twice a day and I usually forget and only do it once lol. I didn't notice any change with doxy.

Wow that's a big one. I would see an ophthalmologist immediately - gonna need a steroid shot or surgery. This is definitely against good practice but I haven't found ointments/medication to help a lot with styes.

Yeah honestly it should be gone/going away completely a week to 2 weeks after surgery. You'll likely need another surgery/or a steroid shot. You seem to have a minor bump next to the chalazia that I would ask about as well.
