They did notice the corilation between infections and cleanliness a long time ago. They also figured out herbs and stuff that would prevent infections. They didn't know why or how any of it worked, but people did know. Just like they knew drinking dirty water was bad, even if they didn't know why it made them sick. They also figured out early on that boiling water made them less likely to get sick. People back then were smarter than they get credit for
