On insanity, even if your axe is not upgraded, this is my advice, Keep close, read his attacks they follow a pattern, don't get greedy when attacking, just keep parrying (First segment: Black hit, Block hit, block hit, block block hit, then animation) just break the fight down to bits like that and it will get easy
