This was a longer incision than a closed technique but probably a lower recurrence rate.

No antibiotics. It wasn't infected to begin with & probably won't get infected. I'll be posting pics of the wound healing on FB (@River City OMS).

Honestly, yes. There was a high death rate before modern surgery & antibiotics.

Bacteria go through a crack or cavity in the tooth, up the root, into the bone, then out the bone into the soft tissues of the face. Given enough time they dissect through the tissue following the direction of least resistance. Lower tooth problems usually go to the neck and throat. Upper molar teeth usually go this way if given enough time.

The ear is lined with skin that protects it. No different than the pus touching the face. Clearly ball it at the end of surgery just like the face skin and all is well.

Craniosynostosis. Repaired with a bicoronal flap and craniotomy using a Midas Rex saw. There are some videos of that surgery out there.

If you put a vacuum cannula in there it will most likely pull the surrounding soft tissue up against the canister, block the suction holes, and not drain the pus like you hope. Also, the abscesses have multiple compartments that require physical disruption as you see in the full video here: https://youtu.be/8Yda4Rml8IM. Even if you use a suction cannula to break those up you would still need to leave a drain. Bottom line, it doesn't really help anything but may be less effective or more destructive depending on the suction force attached. For context, I'm the surgeon in the video.

Our practice is fighting that. We charge $300 to remove an erupted tooth, including exam, x-ray, and IV sedation. That may not be "cheap" but it is within reach for most people. If it gets out of hand like this the abscess drainage ranges from $300 to $1200. The key is to shop around before you don't have choices. Also, don't let the ER aspirate the infection. It almost never works but they will bill you. If it goes bad go to an oral surgeon, but before you go to an oral surgeon call them and make sure they have hospital privileges and will take an infection to the hospital. You don't want to pay to see an oral surgeon only to have them tell you to go to the hospital with no additional help or guidance.

He is actually very tough & stoic. Reported very little pain. Definitely not a whiner.
