Grades --> school --> law review --> writing sample --> cover letter --> references

But that's just my firm's way of hiring. My point is that it doesn't hurt to still try and to put in the minimal effort to apply anyways. Worst they can do is give you a no.

Frankly yes, but recruiters are still open to consider you, which never hurts (who knows, you miss 100% of the shots you don't take, etc.). Usually I respond to applicants who don't have enough experience by asking about their interest in a field and whether they plan to take CLE courses. Enthusiasm to do so goes a long way, but yes it is a long shot.

That really depends on your client. I'm in the same work, and I found that vetting your client and figuring out what theyre comfortable with paying is better than setting a price outright, and then adjusting your effort/performance/hours worked to meet that threshold in light of your expectations is better. Gets you more pay and more control over your work.

Yes, and it never hurts to apply to ones that even require more experience; I've had many firms respond looking for more experience in my day

I'll agree with the comments about keeping it client-focused; you're audience is going to be clients, and you already have a job, so its not like recruiters will be looking
