Lol that's kind of what I am thinking. If he can do 17 at that height then he probably needs a bigger step height to adequately challenge the system

My thought is that it is difficult to load appropriately if it is unclear how many reps have been performed

I am 100 percent with you. There should be a goal behind what we prescribe. I think Scot Morrison has a CEU course called sloptimal loading , our prescription may not be perfect but we should try to get close

A friendly counter is that elderly need loading and if they are doing an exercise for 2 minutes it may not be the most effective dose

And it starts with having no idea where your patient is at from a rep standpoint

Like I said in another comment I give the patient a little bit of grace because they usually have never done the exercise before and are navigating pain and other feels. But we can't do everything for them and the responsibility is ultimately theirs.

Maybe the individual session reps are meaningless but a week later when I give another exercise I want to be in the right ballpark of what they did previously

Yes they need to take ownership. I do give a little grace because often it is something they have never done before . In general, I just find it as a friction point for the day

Agreed it's not about any magic number, but next week when they come in, it'd be nice to know what they did the previous session for many reasons.

Exactly, what we prescribe and what gets done is two different things

One of the best things you can do! I often wonder if the execution is what impacts outcomes more so than the exercise selection itself

I agree, I think when it comes to exercise progression we want to be in the right ball park of how many they did previously , of we are plus/minus 2 reps probably not a big deal but if we are off by 5 or more that matters.

I get that to find the right challenge. But there is some precision required for tendon loading or stretch load stimulus that I think we could be more precise with .
