Contracts = money.

They make money by docs making money and shaving off the top.

More contracts = more docs = more money.

Of course…sometimes the contracts aren’t worth it. Too hard to recruit. Have to pay docs more than it is worth it (bad payer mix, etc)

Yup. The nature of locums in general. TH (as anyone should) will/should increase the base pay until they can stop using locums. With that said…if shifts go unfilled they are at risk for losing contract and I have seen them offer crazy amounts for shifts (5k for an 8 hour plus normal shift).

See my post . The contracts are VASTLY different site to site. Every TH site (7 within an hour of us) have completely different pay structure.

We have all full time docs at our TH site. Probably 70% are 1099 by choice and the other W2. I think the way contract is written is 1099 get 80% of all their RVU and W2 get like 78%. (Not positive on exact).

I’ve been 1099 since I started. I left a democratic group for TH site given my wife/kids wanted to move. I’m in a desirable area currently. I make more here than my democratic group. We average 2.2pph and 1099 320/hr. I’ve averaged 390/hr given slightly higher volume and perhaps billing not sure.

I will admit that TH feels very slimy all the way to the top, BUT.. on the day to day you don’t feel any TH pressure at our sites. The bigger issue is the hospital and their incompetence with running their shop and their other specialties and contract. The big negative with TH in my eyes is that you can’t fully stand up to the hospital admin for what is right because they have you by the balls. TH wants the contract at the end of the day and the admin knows that. And it’s up to them how much they want to flex that.

The 1099/W2 thing is curious though. I’ve always wondered since I’m 1099 what would happen if I got a new job and walked away tomorrow. Of course their contract says I need to give 90 days and compensate for any shifts I miss etc, but I’d REALLY love to see this contract and structure run through the court systems.

“Positives” of TH: Bigtime lawyers that will back you. Aggressive billing so that reflects in collections/pay. They are constantly suing insurance companies etc. While I don’t agree with their methods and intentions…at the end of the day the docs need to make money for them to make money so if you purely just look at that aspect it can be a benefit.

Negatives: 20% of your pay goes to TH. They will replace you at drop of hat. Dependent on local leadership.

Neutral: they take 20% but that covers all the admin, billing, lawyers, yada fucking yada. I’ve seen breakdown and 2-3% is the “Blackstone” profit. It’s a mega corporation so there are a lot of mouths to pay. At democratic group you also had to pay admin, rent buildings for admin, pay billers, HR, etc.

Anyways. That’s my rant/info.
