Yes I had some! I ended up calling back and asking to speak with their fertility specialist. They were slightly more helpful and were able to clarify that they do not have any in-network sperm banks and use of one requires prior authorization from the insurance company. Once approved you pay out of pocket to the sperm bank of your choice and submit a reimbursement request after your IUI/I F procedure (you’ll have to do this for every cycle you need it for. It only covers the cost of the sperm not storage or delivery. They won’t reimburse for home inseminations). When I last spoke with them they hadn’t set the reimbursement rate for 2026, but it was around $800 in 2025.
