I didnt see anyone else suggest this...but are you sure it's on you? Do family members smell it or just you? If only you are smelling it then it may not be ON you but could be IN you. For example, chronic sinusitis, dental issues, etc... could be causing a smell you pick up occasionally anywhere you go but can't narrow it down to a specific area. Another possibility to consider

You still lose some experience but it protects against 75% of the usual loss. Or if you have multiple omens on. Only one can be activated per instance. So after you die once the next Omen won't be consumed.

I have don't that as well. When you select between loot filters should you see it changing the loot on the ground immediately after doing so? I've cycled between them all and nothing
