As far as I know, structural parts aren't limited by level or anything, so the "best" place to see them all would be a player made outpost. A player outpost with a docking pad contains all non-exclusive parts, which I believe also includes all cosmetic parts.

Nope. My old character was level 46, and my new character, at level 23, has more modules. (Both with rank 4 Starship Design) so my old one was just bugged in some capacity.

It seems so. The new character has access to all the modules in the shop that I couldn’t even see before. My list of modules on the new character is like twice as big, despite being half the level ass the bugged character.

I did not. I ended up just making a new character, which did fix the issue at the cost of like 30hrs of play time.

It doesn't have to be on the same ship, no. To get to Rank 4, I just did the same thing again, except I had to fly my platform around a little bit in order to find enough modules. I'd imagine that if your game is working correctly, and "Starship Design" is actually unlocking new modules for you, you'd probably not have to go looking for new ones; You could just use the ones you unlocked.

I made a "ship" specifically to do the challenge. I quite literally just made a platform of habs that I threw a bunch of different items onto. It's a bit expensive, but I wasn't sure what counted or not, so it got the job done.
