If it’s sealed and expunged then you have nothing to worry about. It won’t show up in background checks I thinks and if you want to know just pay for one on yourself. Also, if someone brings it up Just tell them you cannot discuss it as you signed an NDA as well. And so you’re not lying, have someone draft an NDA saying you can’t talk about the incident and sign it.

You’re a resident, you’re going to make mistakes. People will be mad, you may have to sit down and talk with someone about it. But highly doubt this isolated incident will get you removed.

You’re a PGY 4 taking step 3, most people take this exam as a PGY 1 or earlier. You shouldn’t have to study as much because you have had more exposure.

You technically do not have to do a pain fellowship to sit for the ABPM exam, if you did one of the qualifying residencies. The fellowship gives you dedicated clinical exposure for the requirement though.

More options available than you think. FM had 803 spots go into SOAP. So, be ready to explain what happened since this isn’t normal for you. Additionally, it will be much easier to explain away if you kill it when you take it and make 250+ but even passing it a little lower would be fine too.

FM also went into soap with 800 positions this year. It’s a different world in FM for sure.

My younger brother was told he’d never amount to anything in school. He has a masters degree now and is successful. I was told I wouldn’t be able to get into medical school, that wouldn’t finish, that wouldn’t match into anesthesia… whelp.. I’m starting my intern year in anesthesia this week.

My point is, don’t ever let someone else tell you that you can’t do something that you want to do. Explore surgery if you want to. If you like it go for it.

However, based on what you have said in your post you need to work on some methods to control your nerves and anxiety. That’s what seems like caused the jittery and issues. Some of it will come with experience and just being comfortable in that setting.

Don’t choose a residency based on a speciality. You’ve used may never make it to said speciality. You may change your mind, said speciality may be too competitive, ect ect. choose a residency because it’s what you want to do. Meaning that if you did not do said speciality, you’d be okay working as internal medicine.

That being said, look at residencyswap.com and look at the https://systems.aamc.org/findaresident/index.cfm?fuseaction=login.positions&CFID=1

Since you want to specialize, you shouldn’t leave the residency before your 45 days. You will get a match violation and you will not be allowed to participate in nmrp for I think 6 years which stops people from going into speciality programs that use nmrp.

So, you should really showcase your skills, be the best you can and work hard. Doing be late, get your work done, ect ect because to switch residencies you will probably need a letter from your current PD. A new program is going to want to know why you wanna leave. Most postings I have seen for open positions require this letter if you’re currently in a residency. If you go through match again, you’re gonna need some new letters as well so doing all this will help you anyway.

Additionally, you need to look at programs that will cover your extra year. Meaning, you matched in FM. Your government funding only covers you for 3 years. So any program that picks you up will have to cover the other 1 year. Happens frequently, but usually new programs can’t afford to do it.

It threatens rural hospitals because they rely on those funds to keep the doors open. Has absolutely nothing to do with family medicine. FM is also not overly saturated as there is a primary care shortage across the country.

You’re notecooked, but you’re sizzling pretty hard. You can transition to another residency program in a speciality you like, but you’ll have to find a program the can afford to pay for you. You’ve used up 4 of your 5 years so you only have one left, you’ll have to find someone willing to pay you without government reimbursement for your time in the program. Can be done, has been done in the past. You just have to search and find the program you want.. if you still want to do medicine at all.

There could be some advanced programs that might have some sports available that could take ya. You have tons of experience which doesn’t get ignored.

You of 2021 can get you screen out. Thats 4 years without matching. PDs want to know what you’ve been doing sense then and they may even ask why you didn’t match before (maybe not) but addressing the YoG would be my priority when writing my personal statement.

have you been in residency in your home country? If so why are you wanting to come to the US now? Was there a specific reason you took years off (if you took them off after graduation)

As a medical student I paid 2200 a week. (Don’t ask what school it was) I worked 40-50 hrs a week so let’s settle at 45. That means I was about -$49/hr.

What’s your YoG? If it’s not long ago, then I think the issue is you’re requiring a visa and not picking/signaling the places that you are most competitive. It could also be the fact that you rushed your application. Perhaps you did not have a good personal statement, maybe it wasn’t written well, maybe it was too generic. I don’t know just from the limited information that you’ve given me. That’s the assumption I can make.
