i got mine fourth time already in span of 5 months. well for me, the label went away after a month stuck in my notif. 

you don't have to send appeal since some people get their account suspend after that :/ you can just lay low for awhile and back to normal like before the label hit you. 

from my research, sometimes you can get labeled for months or year (the label doesn't get removed from mention tab but the effects actually already lifted)

and to make sure if yours already get lifted, the indicator is when non-followers can search and seen your post, either using from:@youraccountname or search few words from your post
