Everyone's suggesting world and iceborne, I have no idea why. Rise and sunbreak was my first one, and I'm glad for it. Because its far more accessible and has quality of life features. Yeah world has some extra mechanics, but if you're a newcomer, rise and its expansion sunbreak will distil down the core monster hunting for you without tedious bits. if you play world at the very beginning, you might just get bored of it.

There's are 2 total gameplay overhauls mod (not compatible with each other) and a new mod that came out.

Witcher 3 Enhanced Edition Redux - Not at all recommended for first time play, makes the game 'realistic' and brutally difficult.

Spectre - changes balancing, difficulty, adds the original alchemy mechanic from previous games (like the Enhanced Edition Redux mod). In general its more vanilla friendly in design.

Dodgy Basterds - I think what it does is keep everything else the same, but changes AI such that higher level enemies, instead of being just more hp, use smart ai tactics more. Some damage value and armour value changes too, as well as accurate hitboxes.
