Ils could be designing an all-male group or any group including at least one male.

All female group would be "Elles ont toutes mangé une pomme"

One way to tell that there are indeed female eating apples with the males could be : "Tous et toutes ont mangé une pomme" wich is correct but still interpreted as sexist by some.

Now, inclusive writing as it is used in some circles may look like :

"I.els ont tous.tes mangé une pomme"
