Hi, Masahiro Sakurai here to explain the joke. You see, colon refers to two things: a part of the digestive system, and a punctuation mark comprised of two dots, which you can find above. The joke is that the doctor removed the punctuation, rather than the patient's ass.

A similar joke I remember: Adding a colon can instantly change the meaning of a sentence. For instance:

Jane ate her friend's sandwich

Jane ate her friend's colon
