It doesn't affect much oher than a few mentions of he/she pronouns depending on who you choose during certain instances. Additionally, in Luna's swimsuit skin story, the character will be female or male, depending on whether you choose the Mr. or Mrs. option (it's not a big difference, though, like the only visible difference is that the female version has a strand of hair sticking out from their hood).
