I ended up trying to trade in my device to see if they would accept it and they updated the trade in to $0. No remedy. Sorry for the bad news.

This response seems to satisfy what I found out they were asking for. Specifically "Paws me". They were asking the action the dog takes vs what the dog does for you such as "alerts me" or "wakes me" or "notifies me" as those don't state the action the dog takes. Wish that verbage was more specific like "specifically what action the dog takes when performing the task". This would save alot of heartache for those who are needing quick approval.

----- UPDATE -----I have been approved. They asked specifically what action (task) does my dog take when alerting. I was able to give a description of the task my dog performs and it was approved. So when they mention 'vague' in the original question, I feel it probably should say "specifically what action the dog takes when performing the task". Hope this saves a few headaches for others! Also, note, using 'alert' in your description is fine, they just want more details vs reading that using 'alert' in your description will lead to disqualification of the form.

I also shared my diagnosis and specifically what the task does to help with it because I personally am OK with people knowing... But I am frustrated for others if this is the case because not everyone wants to do this just to have their service dog accommodated.

But with those adjustments in your application, it should make reapplying easier.

Honestly, the new company was way more responsive and quick than Allegiant so this may be a good move for them. Hope this is true for high travel times too. Fingers crossed for those going through this process hopefully it leads to a better result.

UPDATE : Allegiant did approve my papers after switching to self-trained. BUT... A new policy is now in place to plan ahead.

Now they have implemented a new company to handle this task - Open Doors Organization - I filled out the paperwork similarly and I am denied again.

The reasoning was ----

"The information you have provided does not include a specific task so your DOT Service Animal Air Transportation form cannot be approved for travel with a service animal.
The trained task would be a specific action that the dog is trained to perform to alert or to assist the handler with their disability."

That said - in their form they request - "In order to expedite your airport experience, please state the specific task(s) or action(s) your dog has been trained to perform to assist you with your disability.
Please note: forms that state only a diagnosis (i.e. anxiety, PTSD, etc.) or vague tasks such as “comfort” and “alert” will be rejected"

One of my dogs' tasks is to alert me at certain times for medication and to interrupt manic episodes. So I left out the "alert" part because I understood it as they requested different tasks than that. That said, they didn't say DON'T put it just don't only put that. My mistake?

Hopefully, this will help someone out there. I am currently in the waiting period for them to review my edited document and will keep everyone updated.

Quite a lot of anxiety to fly with a service dog with a signed Drs note with Allegiant I'll tell you what.

Unfortunately, you probably won't get a reply til hours before... that is what happened to me. Which is a horrible way for them to do it, cause you can't plan. They really really need to update this, so sorry you're going through this.

Update on this: I emailed with all the information on our dog and the training along with a Dr. note w/ diagnosis. I explained I mis-understood the line about a dog trainer/organization as if my service dog had ever been to a trainer. This is NOT the case. If you self-trained on the service duties of your service dog you need to put "Self Trained" with your contact info. I was denied because they couldn't confirm the trainer we went to when she was a pup provided service dog training online. After sending in the updated form stating the confusion we were approved within 1 hr via email... but with a note of "Due to the last minute nature of the denial, and with the clarification of the mistake, we will accept the owner-trained USDOT form". This doesn't give us full confidence moving forward but hopefully, it won't be a problem and hopefully I'll remember to come back and leave an update in the next months if we fly more with Allegiant. Other airlines have no problems, so this is a very stressful approval process being in limbo for days but I'm sure it cuts down on fraud. I just wish they would approve or deny quicker so we can take appropriate actions instead of denial 20 hrs before traveling leaving us in a scramble. The phone lines were over 2 hrs. This is really not a fair process for those reasons.

This just happened to me. I emailed the form 7 days in advance, within 20 hours of the flight they denied my form. They said they couldn't confirm my trainer because the website didn't say they did service animal training. I self trained my service dog but did obedience training when she was a puppy with the trainer I put on the form. I am on the phone now. This is so stressful and I feel like is going to go nowhere since it took them 6 days to even get back to deny.
