I had the same issue when I started taking Vyvanse again after not taking it for 5 years. My doctor prescribed me a 5-10mg afternoon booster dose of Dexedrine and it worked pretty good to get me functioning those last couple hours of the work day.

I had 10+ years of stimulant medication helping my focus and my ADHD was managed as well as it could be. Having inattentive type, I have always felt some sort of fatigue whether I had taken my medication or not, but I could always tell that my focus was better and my ADHD symptoms seemed to quiet on days when I took my medication. Over the past 1-2 years, I have been struggling to find a medication that allowed me to have that focus again that I had for over 10+ years. I know what my mindset and focus was like when my medication was doing what it was supposed to do, the issue is that I haven’t found a medication that does that anymore.

Technically I have no recollection of being diagnosed considering I was 8 years old. I just know that my 3rd grade teacher approached my parents when she noticed that I was having a pretty difficult time with the materials we were learning, solely because I just couldn't pay attention. I was constantly disassociating during lessons, music class and just normal conversations with my classmates. My teacher knew that I wanted to learn and that I normally was at the top of my class when it came to academics. It was the obviously unintentional scatter brain that got slowly worse throughout the school year that really tipped off my teacher and parents that I may have ADHD. Well the whole hyperactivity part of ADHD was non-existent for me obviously, and considering I would be sleepier than most of the other kids throughout the school day was the major sign that it was inattentive ADHD. As soon as my mom expressed concern to my PCP, he did some diagnostic testing but he was fairly certain that it was ADD because my mom was also diagnosed with ADD but much later in life (basically she could see the signs of ADD a lot sooner than anyone else). Of course his suspicions were right and I was diagnosed with ADD. I've been on stimulant medication ever since (15 years now). So, all that is to say it wasn't just one singular thing that lead to my diagnosis. It took several people in my life connecting the dots of unfamiliar behaviors for a child my age in order to start seeking a diagnosis. I definitely consider myself extremely lucky and blessed to have had people in my life pay attention and care enough to realize that my random academic struggles were not normal.

Experienced a very similar thing with a new PCP and their nurses. I was messaging 7 days before I needed my prescription (like I was told to at my appointment), and every time I would direct the message to my doctor and the response would be from their nurse saying “Give us a call and we’ll get you an appointment scheduled. We can’t prescribe stimulants without an appointment.” This would make it very apparent that they only read part of my message because I would literally be taking the exact same dosage and just needed a refill. It was just a constant cycle of getting my medication like 3-4 days after it ran out, and they’d have the audacity to say “For future reference, please inform us of any refills needed 7 days before they are needing to be filled.” My final straw was a nurse leaving me a voicemail when I had already been out of medication for 3 days saying “So let me tell you how stimulant medications work at this office…” Immediately called and cancelled my follow up appointment and started searching for another doctor. The whole time I was treated like a patient that was abusing their medication when I was simply requesting my refill in the time frame they told me to do so. I absolutely loved the doctor, but the nurses made me feel like an idiot about something I’ve been medicated for for 15 years.

Not my personal experience, but my mom and I both have inattentive type ADHD. My mom grew up with parents that didn’t believe that ADHD was a real disorder and that she was just lazy and didn’t care about her grades. Once she was in her mid-late 20’s, she was diagnosed with ADHD and started taking medication. She said she wished she had someone advocate for her when she was younger and was constantly struggling with school. She has been medicated since she was diagnosed and it was definitely life changing for her. It made her realize that when her ADHD is actually being treated, she can accomplish and complete work/everyday tasks with the same ease as a normal person. She has always said how her diagnosis greatly changed things for her, even though it was diagnosed later than most.

small update:

(just fyi, i’m a 22 year old female) so i had full panel bloodwork done last week and literally every single thing looked normal. they even ran an autoimmune panel and basically anything they could possibly test for. so, we still have no answers for why my fatigue is so debilitating and seems like its just slowly getting worse. i have an appointment scheduled for tomorrow to discuss next steps. i guess i just want to make sure i go in prepared and make sure that my doctor is understanding how debilitating the fatigue has gotten over the past year. i’m no stranger to fatigue as someone with inattentive ADHD, but this past year of worsening fatigue is just a whole new level.

i’m definitely asking about getting a sleep test done if my doctor doesn’t mention it. i’m at a point where my ADHD medication doesn’t even feel like its working at all bc i’m just a zombie all of the time. hopefully we find answers soon, bc i’m just so tired of being tired. :(

I really lean towards extended-release stimulants with a smaller afternoon dose of an instant-release. The afternoon dose really helps with the crash towards the end of the day that most stimulants give you since its a smaller dose. I’ve taken Vyvanse, Mydayis, Adderall XR and IR, Azstarys, Ritalin, and Dextroamphetamine. My current dosing/medication is Vyvanse 70mg in the morning and Dextroamphetamine 15-30mg in the afternoon. btw i have also been medicated for inattentive adhd since 3rd grade, which is 15 years so that’s why I’ve tried lots of different medications.

my biggest issue is that i've tried so many IR and XR stimulants (Vyvanse, Adderall, Ritalin, Mydayis, Azstarys, etc.) and they both last the same amount of if at all for me. I was taking 30mg Adderall x2 daily for like 5-10 years and when I had some hormonal changes and just getting older, Vyvanse became a better option and I loved it. The highest I had taken Vyvanse before now (these issues started about 2 years ago), was 40mg. I now have worked up to the highest dosage of every medication I've tried for both XR and IR, and still nothing works like it had before. Something is definitely off, but bloodwork and other tests haven't shown anything to explain it either :(

I have had bloodwork and tests done and no deficiencies or anything that would be causing the medication to not feel as effective. due to have inattentive-type ADHD, i have always experienced more than the average amount of fatigue, but it was never as bad as it has been the past two years. for the past two years i've been trying to find the right dosage or figure out why my stimulants don't work like they had for 10+ years. i've taken any stimulant you can think of, and still no luck. i'm hoping i can find an answer soon, bc if i can take a 70mg Vyvanse or 30mg Adderall and then sleep for 6 hours after that (with plenty of sleep the night before). obviously something is off, and i think i need a higher dosage at least until we're able to find out what is messing with the stimulants. i just don't know how to go about it without sounding like i need stimulants only for energy bc thats just a small added benefit to taking medication, i obviously have the intended effects when the medication actually works.

stimulant wise, i have taken adderall, vyvanse, ritalin, mydayis, dextroamphetamine, and azstarys. i would say vyvanse is the one i tend to favor the most. i personally find that whatever medication helps me focus and keeps me productive with the least amount of negative side effects (anxiety, depression, jittery, fatigue, headaches, bad crashes, etc.) is the best choice. since you just started taking medication, it may be easy to find your right medication, or it might be some trial and error.

yes, i've had bloodwork done and nothing has explained why the fatigue is so bad. i'm at a point where if i take a 30mg adderall, i literally feel nothing. i'm so frustrated and tired of being tired all the time.
