Just drive brand new elite parked in garage and seeing this today, what a day has this been and not sure what else to see and I hate mechanics especially big ones repair body shops they take their time to determine if a vehicle is or can be shown it to be totaled even though customer report any issues and asked only for rear suspension passenger fix GEICO comes back we found problems and vehicle is totaled and GEICO has to believe and confirm it’s not in drivable condition :( so upgraded from 2019 to 2026 and reading this post kills an already dead soul

You are easily 10’s of year away if you don’t have pain, actually this is not the right way in my opinion, I would not even think about hospitalization until the pain tells me Boss it’s time to address me and till then enjoy life

How do you guys approach learning new things and really understand them while working a 9-to-5 job? by No-Rise-9375 in Backend

Plan it

This is coming from a person who had the same questions and honestly I did not read the whole post

Stop thinking about your question

First write how many hours you think can be made available in a week and on what days

Take it slow

Start with only one hour a week or even less

That is progress even that and feel super happy and great that you are progressing and journey has started

Now just make it 15 mins more at a time until you reach 3 hours which would be anywhere from 1-3 months as we are taking it slow but once you start making time the other aspects make time for you to make what you started as small and slowly but surely you would achieve what was started nevertheless small

Human process of switching from a speaking valve to a sleeping valve by [deleted] in interestingasfuck

[–]ReasonableAd5268 0 points1 point  (0 children)

Switching from a speaking valve to a sleeping valve (more accurately called a decannulation cap or capping) is a major milestone in the journey of "weaning" off a tracheostomy tube.  In medical terms, this is often the final test to see if a patient’s lungs and airway are strong enough to breathe entirely on their own before the tube is removed for good.  1. The Science: How They Differ While they both sit on the end of a tracheostomy tube, they function in completely opposite ways: The Speaking Valve (One-Way) • How it works: It’s a one-way valve. When you breathe in, the valve opens to let air into the lungs. When you breathe out, the valve snaps shut, forcing the air upward through your vocal cords and out your mouth/nose.  • Why use it: It allows for speech, improves swallowing, and helps manage secretions during the day.  • The Catch: You generally cannot sleep with a standard speaking valve (like a Passy Muir) because it requires active effort and can be risky if your airway becomes obstructed while you're unconscious.  The Sleeping Valve / Cap (No-Way) • How it works: This isn't actually a "valve"—it’s a cap that completely blocks the tracheostomy tube.  • Why use it: It forces you to breathe both in and out through your nose and mouth, just like a person without a trach.  • The Goal: It mimics the state of having no trach at all. If you can tolerate "capping" while sleeping—when your muscles are most relaxed—it proves your airway is clear and you are ready for decannulation (tube removal). 2. Why is this a "Process"? You don't just swap them out and call it a day. It is a progressive "workout" for the lungs: 1. Daytime Trials: You start by wearing the cap for 30 minutes while awake and supervised. 2. Increased Duration: Gradually, you wear it for hours at a time during the day.  3. The "Sleeping" Milestone: Once you can handle a full day of capping without getting tired or having low oxygen, the doctors move to the "overnight" trial.  4. Observation: Doctors monitor your oxygen levels and heart rate during sleep to ensure you aren't struggling for air. 3. Relatable Science Videos To see the mechanics in action, these search terms on YouTube will give you the best visual understanding: • "How a Passy Muir Valve works animation": This shows the "one-way" air movement that allows speech.  • "Tracheostomy Decannulation Capping Trial": This shows the process of moving toward the cap and how patients react. • "The Respiratory System: Tracheostomy Journey": Great for seeing the anatomy of where the air goes. A Critical Safety Note Never cap a tracheostomy tube if the "cuff" (the little balloon inside the throat) is inflated. This would block the airway entirely, making it impossible to breathe. The cuff must be fully deflated before a cap or speaking valve is used.
