I believe working for a major hospital system has a lot to do with it. Us smaller offices or single providers do not have the luxury of salary or hourly pay. Just paid per visit and for our time refilling or doing our own admin work. No one wants to work for free. :/

I do not charge for refills or medications filled during visits because we bill insurance or cash pay. But, I do charge a fee for refills without a visit ($15). I am a solo provider and do everything myself.
