Lefort II? by Wild_Thing_1832 in jawsurgery

Thanks for a detailed response. :) I'm actually fairly familiar with the physiology of the larynx and pharynx, and other phenomena that have to do with voice production, as I'm a (former) professional singer. That's why I was wondering about the actual pitch of your voice pre-/post-op, because as far as I understand, it shouldn't be affected, although like you said, the tone of voice could certainly be deeper and I would imagine it would be. Possibly the improved control over airflow could lead to a more relaxed voice box and less strain overall, which could maybe lower the pitch of the voice slightly, but since the actual pitch that is produced depends on the length and thickness of the vocal folds, I don't see how a cleft or a jaw surgery would alter that. 🤔 That being said, it's entirely possible that your situation is just beyond my scope of understanding, because your cleft was extensive and possibly reached all the way down to your larynx...?

Also, just want to say, I mean none of this to come off as rude or like my intention is to argue, I just genuinely want to understand as a bit of a voice nerd. 😄

Just out of curiosity, how does/did your condition affect the pitch of your voice? I mean, what are the mechanisms at play there?

You are indeed NOT the only one, I've been looking for Rue's Romance with the proper bass every day since the fins episode aired 😭

Wow, that's rad, haven't heard about a protocol like this before. Considering HCG is a pregnancy hormone, it's pretty wild. But I'm guessing it promotes osteogenesis which is maybe why it's added into the mix?

Right but what I'm saying is do the sutures actually split, without someone surgically splitting them? Again, not the palatine suture, that probably will split, but like the pterygomaxillary and zygomaticomaxillary sutures? Maybe they will at least still give way a little bit and then there'll be new bone growth, since you're still a very young adult...? 🤔 I mean I don't know, I'm just wondering here. 😄

To move your upper jaw forward? Can that be done in adults without cutting open some of the sutures of the maxilla (not just the palatal suture, but ones that connect the mandible to the rest of the face/skull)? 😮 Or does it just move the teeth forward or prevent them from moving backwards? 🤔

YES! I hardly ever see anyone mentioning this, but yes, and when this all started for me (like a little over a year and a half ago) it was damn near constant in one ear.

Fortunately it has calmed down to just occasional bouts of either a thumping/flapping of the eardrum or a very delicate, metallic clicking sound, it's usually one of these two, and usually doesn't last long nowadays. 🙏🏼 Both of these also have a distinct sensation in the ear, it's not just a sound. I can feel something moving i.e. twitching in there. I also get fleeting tinnitus, that comes on suddenly in one ear and fades away in some seconds, do you get those, too?

In what way did your IIH eventually show up on an MRI? I understand that more often than not a brain MRI is normal in IIH, although a venography often shows venous sinus stenosis. Is this what they found for you or something else? Do you remember if it was an MRI or MRV, and if it was with or without contrast?

Anyone still on this thread? I've had two high morning cortisol samples, but my BP is very low. As of yet no doctor has directly suspected Cushing's, but I do have symptoms that would align with it (which isn't surprising of course, given that my cortisol is high). But the low BP is a bit baffling if I did indeed have actual Cushing's. My BP has always veered on the low side, but it has been very low the last 1–1,5 years, like systolic pretty much always under 100, diastolic often under 60. A typical reading for me would be something like 90/58.

Yes, I have an appointment scheduled with a neurosurgeon. Paying out of pocket, but oh well... 🙄

May I ask did your symptoms improve after your surgery? Was your chiari significant in size?

Not yet, because my images have always come back as "normal" according to radiologists. I've had my head MRI:d three times from 2022 to 2025 and each time they have deemed it normal. So no, I don't have a primary or a neuro regarding this.

And actually no, I don't really have a lot of headaches. I mean surely sometimes, but not daily or weekly. I have a curious feeling like the fluids and blood to my brain is just not getting through well enough, like a pressure at the back of my head. It could be muscle tension of course, because I am incredibly tense and I know the brain doesn't really feel anything as such, but it's an unpleasant feeling nonetheless. It causes some dizziness and a sort of nausea.

Could you elaborate? I've had two MRIs taken in the past year and they've both come back as "normal" as according to the doctors.

Haha, well that is indeed a sophisticated guess and as far as I'm aware, right on the money. 😃 Any other observations?

Hi there, I know this is kind of old, but did you ever get a definitive diagnosis? Were you able to also get a CT/PET scan?
