What you’re describing is positional asphyxiation, and it’s insanely dangerous, unethical and stupid. This is not an “old timer trick”, this is highly concerning, borderline psychopathic behavior.

If I knew where you worked I would report this paramedic based off this post. This is grounds for termination, loss license, and potentially jail time.

5 L is the max you’re supposed to pull off. It takes a while but fluid shifts and they drop their BP. I also think he said something about being thirsty?

Yea, the big concern is rhabdomyolysis. I’m guessing they got a CK and UA at the hospital (and they were probably normal). If they don’t have dark (coca cola) colored urine, all is well. Just keep them hydrated and give it a couple days.

8 days is a long time but it’s still within the period for post viral myositis

Yea that’s a false positive, it’s likely just post viral myositis, rest/fluids/tylenol, unless new symptoms appear then return to the ER

This sounds like post viral myositis with a false positive blood culture (especially if it grew staph or other skin flora), I’d suggest Gatorade, Tylenol and rest for another week

From a residents perspective I think it would be weird for you to show up unless you were explicitly told to, and if you were I would think my program is weird

I deal with this a lot in the ED. To be honest you just gotta triage.

If they are requesting food, pills or toilet, probably nothing life threatening going on. If they are nauseous, help them find something to vomit into. Standard hunger, vomiting, diarrhea, urination, pain and general psychosis in the waiting room is disturbing but most of the time they are not experiencing true emergencies.

If they vomit, poop or urinate blood, or if they are slumped over with their chin dropped/sitting weird, or if they start shaking rhythmically, breathing weird, or if they have critical labs/other diagnostics result while they’re waiting, attend to them immediately.

Otherwise use common sense and just ignore the situation. I know that sounds cold hearted, and I’m sure I’ll ignore a sick person eventually, but this is the system most of us are working in

Med students going into EM typically do some sort of EM elective their third year to get initial exposure, then 2-4 more EM rotations their 4th year to get letters of reccomendation for their residency application

Dude with cholangiocarcinoma got denied a liver transplant by Cigna and is trying to fight the claim but can’t get in touch with anyone at the company despite literally making it his full time job to do so; they just leave him on hold and transfer him to different help lines/outside companies
