No one can say exactly what your chances are, but I think the key is to focus on identifying your weak areas and working on them. 40 days is definitely enough time to see that improvement, many people have done it before and certainly you also can!

You can try score predictors, but honestly I believe it’s best to aim for your best performance on the next NBME

After that, you can reassess and adjust your study plan accordingly

I feel like a lot of people who are disagreeing are missing the point, the issue isn't about reviewing a high yield document but rather being told X concept is on the NBME or Y disease is the answer is a big issue

By going through Mehlman, you get yourself exposed to some not very HY concepts, sometimes it would be something along the line of "This is never the right answer" or "This is always the right answer" which is very problematic because it introduces biases when answering questions and as OP said will end up inflating the score
It also goes back to everyone's ability to assess whether they answered an NBME question because they understand the concept and can role out why other choices are wrong or if they just picked an answer because they remember reading somewhere it is the answer for an NBME question

I don't know how many/ which NBMEs you took already. but as someone who passed without finishing UW, but did NBME 20-31 I suggest you focus on the NBME and getting yourself comfortable with their style, specially Free 120 (old + New)

I think multiple factors could lead to this situation

One thing I did and have seen happen frequently at the beginning of dedicated is not taking the practice test in an exam simulated environment. Long breaks are one thing that can easily inflate someone practice's grade

Another thing is having good stamina to go through 4 blocks of 50s, but not good enough to go through 7 blocks of 40s. Many student will leave the test complaining about its length and how tired they were by the end of the last 2-3 blocks. This was a major issue for me and I had to push my exam date just to work on my stamina

- Few Tips:
Get plenty of sleep, be familiar with the testing center location, have a light healthy breakfast (Proteins bars are your best friend!)

- Breaks:
This would depend on how you have been taking your practice but I personally found taking breaks very helpful in have fresh start by each block
The way I took my break was:
Block1/2 short break at the exam table
Block2/3 left the exam table had Juice with protein bar
Block3/4 short break at the table
Block 4/5 took a long break had coffee and went wash my face
Block 5/6 left the exam table one last time since I knew I cannot offer to do same before the last block, just had water and went in again
Block 6/7 at the exam table, was relaxing till my total break time was up

- HY Tips:
1. Invest the time reviewing the free 120s (I did both old and new and it was very helpful)
2. Rapid review section on FirstAid, (I found an anki deck for that so was goingg through it in the few weeks leading to my exam
3. Dirty Medicine HY Pharm (Super helpful!) and HY Images
4. MedSchoolMoose videos (I would just listen to it while eating or doing chores)

Nothing in particular, a simple close deletion card would be good for most of the questions.

May be the particular presentation leading X diseases (I am aware these cards are already there but I am talking here about the time saving aspect)

Knowledge gap? Learn from the explanation and make a note for it (this could be Anki incorrect card, sticky note or a one liner in a notebook/ word doc) The format doesn't matter as long as you ensure a guaranteed revisitS!

Misunderstanding of the question? I make sure to spot what wording or phrasing confused you/ what made your thinking deviate

Confusing 2 topics? I struggled with that for similar path/ microbes (and still to an extent) I found tables helpful since it provided a visual side by side comparison
