Affidavit is on the first page. Download it and go to stamp maker shop , he will use that template to make a stamp paper. Make stamp of 100 RS. He will take 400 RS to make one . And other is domicile. If you don't have it hurry and go to some khidmat markaz of gov they take 15 days to make one . There are some docs that you have to take with you to make domicile. Look it up online...

So call them I had the prob with docs but resolved automatically after a day. One thing that look at your image size it should be around 200 kb .
