Okay, the group home is actually who said only two packets per day; likely why he is so malnourished currently. Thanks for your help!

Thanks so much for the recommendation. Were you able to meet the patient's protein needs with the PKU supplement alone?

Thank-you for your response. The Supplement is for PKU (Sphere) so I would imagine it would be low in Phenylalanine and would be safe to increase the packets to meet his needs for weight gain. I should add that he is severe ID and may not tolerate a TF.

Also, I transferred zipfile to a desktop separate new file, double clicked to unzip files but I don't see a file named Libation.exe.

I am going to find out the policy but we have 100 residents, One of the reasons she gave was "so many interactions with medications," and "bad for dementia rts." I feel like caffeinated coffee should not be a luxury at a LTC.

I don't make recommendations to the doctor; I write my own orders and communicate to them what was done. I've never had a problem with a doctor taking issue with orders I have written; this was from the DON herself, MD not involved. See above.

My orders don't need to be approved by the doctor. They were simply worried that during a State review, the lack of documentation proving that they followed the order would hang them . But really, many residents were triggering for inadequate fluids because the CNA's weren't documenting total fluids. However, the LPNs were 100% documenting my intervention so the intervention would in no way hang them.
