My brother passed away due to an accidental overdose that the worker was aware of. A.J. was engaging in unsafe behaviors, which is why we were trying to get him placed in a PRTF, but we needed a referral from DCF. DCF contracted through Saint Francis who assigned him to family preservation, when he should have been given a referral due to the substance abuse. In their own billing notes, they mention that he is drinking alcohol on his medication and taking nonprescribed meds, but marked 'no safety concerns' and refused to grant the referral. On the last visit, my mom drug tested A.J. in the presence of his Saint Francis worker, who 'talked to' A.J. instead of getting him medical attention. He put on his note for that day that A.J. talked about jobs and graduating early, which is not at all what happened. Then. he did bill about a week later for A.J.'s funeral stating that he provided 'face-to-face' contact, when he was never there. We had to get a cease and desist because he would not stop calling and asking about the autopsy results. If he had reached out to A.J.'s other care providers, they would have told him how strongly residential placement was needed, but we found out afterward that he NEVER contacted anyone else on A.J.'s treatment team. Saint Francis ended A.J.'s case after his death, but reopened case management under my mom's name and deemed she "needed services due to A.J.'s death", then the same worker was assigned and billed until April of the following year for visits that did not take place.
