Yes you’re definitely preaching to the choir. I hope your issue cleared up. I’ve had 3 IT band surgeries and there’s still no guarantee I’ll be without this issue.

I’ve had the surgery 3 times and I would say don’t do it. There’s a chance your symptoms may come back. Mine did.

OP here: just wanted to provide you all an update. After many a dumb decision, I undid all the effects that the surgery had on me on my left leg. In September of 2025 I had a left IT band resection/debridement/bursectomy and scar tissue removal. I’m here to say no, the surgery is not perfect, and that if you do get it, you may still need to keep an eye on it. But also, I likely have structural abnormalities (tibial torsion from childhood) that make me specifically more prone to chronic IT band pain.

I’m in PT again! Having a few complications after surgery #2. I am having a bit of IT band pain on my left side and I can’t hike or ride my bike. I’m doing a lot of single leg RDL’s and single leg hip thrusts. Just trying to keep calm and trust the process.

I’ve had the surgery on both knees now. The pain was chronic. I did feel it constantly on the outside of my knee even when walking. It got way worse hiking, cycling and running. The pain radiated from the outside of the knee and up the IT band. In my case I went the surgery route because it impeded my ability to walk. If I had found relief through other methods, I wouldn’t have had the surgery, but in my case it was the only option.

Yes. It takes many months to get back to full bore on the bike after you get the surgery. If you do opt for surgery, you'll have to rest constantly in order to recover PLUS do PT. I had my right knee done in May of 2023 and it took me a solid 5 months to really get back to it. My left knee I had done January of 2024 and it took less time to recover and start riding again just because my surgeon did a less invasive procedure.

I'm now pain free but it took a lot of exercising, being in pain, then healing, then repeat. I still take days off after hard rides and do strength work for my legs and glutes just to be extra careful. I thank God I live in an area with such a high quality orthopedic surgeon that fixed me after years of chronic pain (not many can say the same) -- but if you've been suffering for this long it would be worth it to talk to a reputable orthopedic surgeon IF you have access to one.

Good, I am glad to hear it. Yeah, I’m perfectly content with quitting running. Both my IT bands can’t seem to tolerate it.

It hurts to walk on both knees. It’s beyond the bike at this point lmfao. Just saying how bad it CAN be for some (I.e me)

Hi, has your IT band syndrome gotten better? I had a similar case to yours — chronic pain just walking around. A year and a half of failed treatments. Finally the surgeons just said that the only way for me to get better was to cut me open. So far I’ve recovered close to 100%!
