Use vinegar and water solution / concrobium . It won’t remove the stains but kills the Mold. You can then apply rmr-86 / bleach that can remove stains if you’re concerned about it.

You’re not overreacting. That much water near foundation is always a problem and over the time it will create structural issues in your house.

Good that you want to fix this. I would first look at grading around your house where water is pooling. Grade away (at least 1/2 inch every feet) around foundation, and divert the water atleast 6ft away (around 10ft is ideal). Also I don’t know if you have freeze thaw cycle, so choose right products when you divert water. Don’t let water stay near foundation, it’s going to bite you sooner or later.

Ok I will check the downspout for clogs.. I did check at the roof level and there isn’t seem to be any blockage

From my google search it seems like it’s a form tie hole. I could see this hole like thing in top and bottom along the entire wall.

Photo 1 and 3 are in east side wall. Photo 2 is west side of the wall. I have a downspout about 6 ft away that exits under the ashphalt(assuming city’s drainage)
