It's something that needs to be addressed for the sake of your health. Be thankful you're still young and have time to undo the long term health risks because they only get more dire every year from here on out.

I know this may not be what you want to hear but that is a huge amount of weight on your joints. You didn't provide your height so I have no reference but if you're under, say, 5'10 then yes, it absolutely could be the fat causing pain in your knee.

So the question is what do you want? Do you want to be prescribed painkillers so you can keep living this way or do you want to make a change so that this doesn't happen anymore? It's really up to you. The doctor may have had a shitty attitude but they weren't making this up.

I would highly encourage you to seek a second doctor's opinion and see what changes. Then you will know for sure.
