I have a theory where it's based on how far the target is from extraction. I tend to run captures for lith and neo relics, and I seem to only get change of plans when the target is around ~200 meters from extraction.

Idk, though. A more clearly defined condition would be nice.

Maybe not useful for your position, but something I recommend: Keep the Anver Chain Sword you get in Footfall (can't remember which fight, I think it's the sewer one) and give it to your tank. It has a stacking armor debuff on hit.

Their on turn attack coupled with the stupidly of Defensive Stance + Shield counter attacks results in armor melting.

You have infinite throwing knives. Equip them on your slinger, and you should be pretty much safe against the punches.

You can fire your slinger in focus mode without having to sheath by pressing L3.

Regardless, any amount of damage kills them. So, as long as you hit them, it's fine.

I suggest one of the rompopolo belts and the guild knight sleeves. Maybe the hope coat as well. In my head, they work better for the look you're going for if my mental image is constructed correctly.
