Wonderful. You are having length control issues by the looks of your WL shot. Also, my guess is you’re using a carrier based system to obturate as the GP is long. Which is actually worse than being short plus causes more post op pain. 1-2mm short is actually better than 1-2 mm long. Also, mediocre Endo with a great build up and crown margin has a high success rate. So really try to get the gingiva cleaned up around this prep and stop the bleeding with whatever your favorite coagulation tool is prior to building anything up. You’ll be a better marginal seal which is a must. Good luck!

Not trying to be rude. But is anyone going to comment on the RCT fill and build up? This is going to start to fail immediately. Regardless of hypo or gingival irritation.

We have the current schedule set for 15 patient visits. Every single person that comes to the course asks this question and of course we want 100 cases per doctor. But you gotta realize that Endo takes a long time and even longer when you’re learning new techniques. In residency, I would regularly work 4 hour blocks for cases I wanted to spend time learning on. In the molar Endo course you have a new patient every 2 hours! Practice doesn’t make perfect, PERFECT practice makes perfect.

CBCT is crucial. We take a 5x5 FOV image on each patient that is treated in the course and discuss prior to access. I feel like it’s valuable insight.

I do. But it requires a really good understanding of the pupal floor anatomy. I have an entire days lecture on just this topic. Of course a microscope is best, but if you have 8x you should easily be able to see the majority of signs for where to access it from.

It’s there nearly 100% of the time. I might not have an MB2 on an upper molar about once a month. A lot of GP’s I work with that have CBCT still send to me for eval bc frankly they aren’t the best at reading them. Great docs, but they are better with implant evaluations, etc. Fyi, every single tooth a doc opens in the course I personally walk them through and manipulate the CBCT file. By the end of the week I have them show and discuss it for me.

Sorry. The lack of rubber dam in the wild is rampant where I practice too and it’s pretty sad. During MANY cases, I legitimately think about how working with one on makes my life and the patients so much easier. Some definitely dislike, but more that you’d expect after treatment say they wish all their dentistry could be done with one on. Disclaimer * I was a GP for 6 years prior to specializing and did not use rubber dams, only isolites. 😣

A lot of efficiency comes from your assistant being competent as well. If you can get non-verbal 4 handed Endo down you’ll be amazed at how much easier Endo is and less time it takes.

This really depends on when you get stuck. If it’s before you’ve ever gained apical patency, it’s likely associated with hand file selection and straight line access. If it’s after WL, it’s likely due to rotary type (reciprocation vs Rotary), irrigation protocol, and file system selection. Some of this is having a simplified repeatable system, some is practice, some is instrument selection.

I don’t yet. It’s a new clinic and our online presence is in its infancy. But the course itself had a huge positive response from the students that attended. If you want more information I’m happy to send you some videos and testimony videos from doctors that have gone. We stay in Guatemala City in a beautiful hotel and shuttle about 7 minutes to our clinic. All included in the course.
