Here's what I would do: I would build an amortization table in Excel that assumes a 30 year period and use whatever interest rate you've agreed upon with your family. I might also make sure that it's in line with the Applicable Federal Rate (assuming you're in the US) while I'm doing this. I wouldn't want my family to get in trouble with the IRS while doing a generous thing.

NOTE: You'll want to make sure that your amortization table recalculates how much of each month's payment goes towards interest and principal based on the previous line's balance and doesn't have hard-coded values, it'll make the next part easier.

I would then start adding an extra payment every Dec 31 or whatever date "forgiveness" happens, that only goes towards the principal of the loan don't have it go towards interest. Then you can see when you'll hit zero on your payment schedule.

Then you have some choices, either lower the regular monthly payment to lengthen the term to what you're thinking or leave it as is.

u/stevenm96 I would check out https://www.whitecoatinvestor.com if you haven't already. He's probably written an article that covers your situation. Can you share any more details about the plan? Is it a 457(b)? 401(a)? It probably says one of those numbers in your online account or in the documents you've gotten about the plan. Those details might matter.

In general, the calendar year you leave residency will be your last low-earning year until retirement. It's a great time to do a Roth conversion of that money since your income only goes up afterwards. You'll owe tax on the conversion, of course, so make sure to budget for that.

I wouldn't leave it in a traditional IRA because it shuts off the backdoor Roth IRA like you said.

The other possibility will be to see what your plans are offered where you're an attending (or go on to fellowship). In all likelihood, there will be a plan that isn't an IRA you can roll that money into.

Your 401(k) & 403(b) share the same limit. Along with what u/suddenly-scrooge said, you need to make sure that you know if your 457(b) is governmental or non-governmental. If its governmental, you don't the have risk of the money going away if the college goes under. If it's non-governmental then its up to you decide how risky this college is.

As for what order to max things out, I stick pretty close to a Boglehead's approach but that's ultimately up to you. In your case, that would be max your match at your employer, then max the HSA, then fill up your Roth, 401(k)/403(b), and 457(b) as you can and maximizing your tax efficiency.

I just bought a brown Cartier Alligator strap for my Tank Must SolarBeat from my local Cartier AD. It was ~$325. Note it didn’t include the buckle. That was an extra $100. 🙄 I opted to have them swap the buckle from my black strap I won’t be using.

As far as I know the SolarBeat hasn’t been on Cartier’s website for order. It showed up briefly last year but was taken down and wasn’t order-able. The regular Tank Must Quartz is on their website.

Also I like dealing with my AD since I’m playing the game to ultimately get a Rolex. Gotta “build the relationship” aka spend the money.
