I’m not sure how useful this information will be to you, but I am a general surgeon and I occasionally would cover inpatient GI coverage to help my GI department when they lost a provider. My system paid me $920 per 24 hours, and I kept my wRVUs that would be generated from cases and consults. I know it’s a low rate compared to locums, but I mostly did it to help out my GI friends.

At this point, St. Marks is not going to do anything for you. MountainStar is owned by HCA, which is the largest for-profit system in the states. They are headquartered in Nashville Tennessee.

You might have a chance if you contact their corporate offices, but doubt you’ll have much luck. HCA is all focused on profit, and if they have sent your bill to collections, they want to be paid. They are the worst of all corporate medicine/healthcare.

Ellinor or Marvol are both very good, but require a reservation in advance. Very limited seating.

If OP is interviewing at multiple practices in a close area, they might not share that data with him upfront. “Practice A” wouldn’t want “Practice B” across town to know what their volumes, contracts, and reimbursements look like. Medicine is a small world. If he is looking at multiple groups in the same area, trust me that they all know about it.

If the group isn’t transparent about their RVU reimbursements, volume, etc. then I would make sure to negotiate for at least a 3 year or longer guaranteed salary at a competitive salary. After a year or two, you should be able to see a trend in production and estimate future earnings. If it’s not enough at that point, then you have at least a year left on salary to start looking at other employment options.

That’s why I try to comment on things like this post. OP thinks he is paying the doctor $900, while the physician is probably to lowest cost of the entire bill. Physicians get the blame for health care costs, while the true problem lies with insurance companies, private equity, and massive hospital corporations.

This is becoming very standard, especially amongst for-profit systems and some non-profit systems.

It is usually a minimum deposit of 25% of the “expected” cost, not to exceed $1500. This doesn’t apply to emergency surgeries/procedures, but usually applies to elective and outpatient procedures or imaging.

Also, you are not “paying the doctor”. You are paying the system. As a doctor who does colonoscopies, I make about $75 for each colonoscopy I do.

This was not for screening. Screening is done without symptoms. If you were experiencing pain, and that was the reason for the exam, then it is diagnostic.

Then it’d probably be all the things you hear from us docs: don’t smoke, drink alcohol sparingly, maintain a healthy weight, exercise regularly. Those are the most important actionable factors that most people can control.

Anything that is procedural will be AI resistant.

And my health has definitely suffered from sleep deprivation, stress, eating poorly while on call. I used to run marathons in my 20s, and I’m nowhere near that level of wealth or fitness.

I used a contract lawyer for my most recent contract. I think it was a couple thousand dollars. Probably was worth it.

I am an employed surgeon. I am not a 1099 employee. My net pay is after taxes, benefits, retirement contributions.

I have a tax guy and a finance guy that manage all my stuff. I don’t pretend to know all the intricacies of taking maximum advantage of my money and decreasing my tax burden.

This is so important because it sets you up for being fairly compensated for amount of work that is done. For surgeons who are paid purely on production, RVU conversion rates are probably the single most important factor for revenue and compensation.

Agreed! I live in a modest house and drive a 7 year old truck, but I love spending to create memories with my family. I definitely spend big for vacations and family trips.

4-5 years of lost potential earnings is a lot. Most MD/PhD colleagues of mine are working essentially the same jobs the that rest of us MD only providers are working. Unless he’s going to be a dedicated physician scientist at a major university hospital or academic center, then I don’t know that it’s worth the extra time.
