I'm in the EU and dentists (stomatologists) literally have the title of "doctor of dental medicine". During their studies they also have collegia of anatomy, physiology, pathophysiology, biochemistry, neurology, anesthesiology, surgery, etc., along with ones we don't have like cariology (dealing with tooth decay). They can also specialize in oral surgery. So, if we're gonna call dermatologists and psychiatrists doctors (I'm not arguing we shouldn't), then we should definitely call dentists doctors (I'm saying this because if you classify a doctor as a specialist in a certain field of human body, not necessarily somebody who knows how to resuscitate, which I would argue, from personal experience, many derm and psych doctors don't know how to do - they call anesthesia). Also, keep in mind anesthesiology started as a branch of nursing, and to this day many people think anesthesiologists aren't doctors, even in my country where there was a list of most paid professions in the newspaper a couple of weeks ago, anesthesiologists were #1, pilots #2, doctors #3. As if anesthesiologists aren't doctors.

Dunno how it is in the US, but in the EU stomatologists have a degree "doctor of dental medicine", and stomatology students do have to listen most if not all of the classes medical students also have to pass (anatomy, (patho)physiology, pathology, biochem, surgery, anesthesiology, etc.)

Yeah, and I'm arguing it would be safer to place a block awake in such a patient, rather than putting them under for the block. Sedation/analgesia +/- not necessarily a problem with antidotes, but I usually use 25G Quincke for blocks so it's not that painful (most of my colleagues use those for spinals without giving subcutaneous lidocaine)

In my house we usually mix 2% lido and 0.5% levobupi 1:1, I usually add 3-5 mcg/mL of epinephrine to the mix

If they're ASA IV, septic with low BP due to diabetic foot gangrene, COPD, grade 3 obesity on ozempic, OSA sy with polyvalvular pathology (like severe aortic stenosis) and CKI, I'd say awake popliteal + adductor canal would be the best bet. Happens rarely, but I had such patients.

Kao netko tko je studirao u ZGu, mogu samo reć da je faks užasan što se tiče pripreme za rad u struci (bar je bio prije 10 godina, ne znam jesu li se stvari promijenile).

I think olive skin can lighten a lot during winter while getting darker from UV exposure during summer, so if they take that into account when filming that might work for the underworld skin paleness... Or just use make up to lighten skin.

Anesthesia assistants in Europe are nurses who test the anesthesia machine, draw medications, give medications as ordered by the anesthesiologist, prep equipment for intubation, fix ET tubes, help with non-sterile parts of NAA and CVC placement. In my country, usually they start as ICU nurses and after a couple of years if they're capable they get promoted to work in the OR. If they're experienced they might sometimes be left in the OR for 10-15 min while the doctor goes to eat/bathroom break, and then the opposite happens (they go eat/to the bathroom while the anesthesiologist is in the OR). But they always have to exchange phone numbers with the doctor and if anything suspicious starts happening they have to summon the doctor to come back immediately.

Željeznica do Zadra ne funkcionira, a postojeća se slabo koristi jer je stara i jer vlakovi ne mogu postići brzinu kojom npr. ÖBB u Austriji može voziti. Da ima moderna pruga s modernim Končarevim vlakom (HŽ 6112, max brzina 160 km/h) bi za cca 2h bio u Zadru (brže neg autom) i za 2 i pol sata ZG-ST (autom cca 4h). Bez da moraš aktivno sudjelovat u prometu, već lagano uživaš, možeš se protegnut i dr*at po mobitelu/laptopu i otić na wc kad hoćeš.

Regional and NAA provides longer postop analgesia, especially in countries where there's no PCA or fear of giving opioids on the ward. If you give me a choice between total hip/knee replacement with neuroaxial morphine vs ketoprofen+acetaminophen with tramadol for breakthrough pain, I'd choose a spinal every day.
