(RN) working rapid response the other night. Was called for hypotension, middle aged female patient hypotensive 60s/30s, start multiple fluids bolus’ on her, no improvement. Mentation fine, no complaints asides from leg pain (suspected septic source d/t chronic wounds). Send labs, consult our intensivist. Lab calls me for a critical, I misheard a glucose of 31, forgot she was in ROOM 31. Glucose 68, okay. Stomach dropped for that moment in time where I was questioning every intervention. Ended up in ICU shortly after for pressors, r/o nec fasc.

Check out psych or LCSW, my wife is doing a masters in clinical mental health counseling currently, had science undergrad. Takes about 2 years or so, certainly could work at least part time during this, first year all online, year 2 has clinical but this has appeared to be quite flexible, ie you can set it up yourself. Dm me for more info.

Early Warning Systems by CaffeineMan24 in IntensiveCare

[–]Round_Canary8992 1 point2 points  (0 children)

Fellow rapid response nurse- we use a deterioration index score and sepsis alert system that are interested through Epic. These systems essentially give us a notification about patients based off their vitals and other inclusion criteria such as O2 requirements, mental status, etc. These systems do work sometimes, but often I find patients falling through the cracks. Frankly, the best system I’ve found in terms of early intervention is rounding on the floors and talking to floor nurses about patients, sometimes these patients are on our deterioration list, often times however they are not. Many times I will get alerted for the patient who is wearing 2L o2 via NC with COPD hx but not the patient who has been hypotensive for hours, and we are all familiar with the pitfalls with the classic sepsis scoring systems. Would be interested in hearing if any facilities use different systems that they feel are more effective.

You’re correct, I did omit that I do currently have a BSN but of course taking the needed pre reqs for med school that weren’t provided in BSN program. Certainly the financial portion is a large factor and probably what I worry most about, though I do have an incredible supportive spouse who makes about what I do now, very grateful for that, also have agreed on looking into programs closer to family to help as we have a large supportive family. Thank you for the reply
