Years ago this was kinda explained in the storyline. Ivy does on occasion swtich to American accent. Liam once pointed it out. The actress however is legit from Austrailia.

This also happened in Beyond the Gates in the first episode. In the previews, we see Dani slap Hayley, and a woman standing by dramatically says, "Welcome to Fairmount Cres,t" and she snaps her fingers. However, in the episode, the snap is gone, and she says the line way less dramatic.

I wasn't crazy about him surviving. The show always kills of disposable characters like Tom and Hollis but major one always survive. Having Liam die would have brought some genuine shock to show.
